摘要:
目的:分析益肾活血方对全麻人工流产术后患者子宫恢复和宫腔粘连发生情况的影响。方法:166例全麻人工流产术后患者采用随机数字表法分为对照组和观察组各83例。对照组采用戊酸雌二醇片、戊酸雌二醇片/戊酸雌二醇环丙孕酮进行治疗,观察组在对照组的基础上采用益肾活血方进行治疗,两组均治疗两周。观察两组治疗期间月经复潮时间和临床症状发生情况,比较两组治疗前和治疗两周后血清基质金属蛋白酶(MMP)水平、子宫内膜阻力指数(RI)、血流参数(PI)及子宫内膜厚度、宫腔粘连评分。结果:治疗期间,观察组月经量少患者占比(2.41%)显著低于对照组(13.25%,P<0.05)。与治疗前比较,治疗两周后,两组血清基质金属蛋白酶-2(MMP-2)、基质金属蛋白酶-3(MMP-3)、基质金属蛋白酶-9(MMP-9)水平升高,观察组均高于对照组(P<0.05)。与治疗前比较,治疗两周后,观察组子宫内膜厚度增厚,且厚于对照组(P<0.05),观察组子宫内膜RI、PI降低,均低于对照组(P<0.05)。与治疗前比较,治疗两周后,两组月经情况、粘连范围、粘连类型评分降低,观察组均低于对照组(P<0.05)。结论:益肾活血方可有效促进全麻人工流产术后患者月经恢复和子宫恢复,促进子宫血清MMP-2、MMP-3、MMP-9水平升高,有效降低宫腔粘连发生的风险,具有较好的治疗效果。
Abstract:
Objective:To analyze the effect of Yishen Huoxue prescription on uterine recovery and intrauterine adhesion in patients after induced abortion under general anesthesia.Methods:166 patients after induced abortion under general anesthesia were divided into control group(83 cases)and observation group(83 cases)by random number table method.The control group was treated with estradiol valerate tablets,estradiol valerate tablets/estradiol valerate cycloproterone,and the observation group was treated with Yishen Huoxue prescription on the basis of the control group,both groups were treated for 2 weeks.The level of serum MMP,RI,PI,endometrial thickness and the score of intrauterine adhesion were compared between the two groups before treatment and after 2 weeks of treatment.Results:During treatment,the proportion of patients with low menstrual volume in the observation group(2.41%)was significantly lower than that in the control group(13.25%,P<0.05).Compared with before treatment,the levels of serum matrix metalloproteinase-2(MMP-2),matrix metalloproteinase-3(MMP-3)and matrix metalloproteinase-9(MMP-9)in two groups increased after 2 weeks of treatment,and the observation group was higher than the control group(P<0.05).Compared with before treatment,the endometrial thickness of the observation group increased after 2 weeks of treatment,and was thicker than that of the control group(P<0.05).The endometrial RI and PI of the observation group decreased,and were lower than that of the control group(P<0.05).Compared with before treatment,the scores of menstruation,adhesion range and adhesion type of the two groups decreased after 2 weeks of treatment,and the observation group was lower than the control group(P<0.05).Conclusion:Yishen Huoxue prescription could effectively promote menstrual recovery and uterine recovery of patients after induced abortion under general anesthesia,promote the increase the levels of serum MMP-2,MMP-3 and MMP-9,effectively reduce the risk of intrauterine adhesions,and had better therapeutic effect.
