摘要:
目的:研究大蒜素对子宫内膜异位症(EMs)大鼠异位病灶的影响。方法:采用自体内膜移植的方法制备大鼠EMs模型,50只模型大鼠随机分为模型组、孕三烯酮组(0.5 mg/kg,阳性对照组)和大蒜素低(5 mg/kg)、中(10 mg/kg)、高剂量(20 mg/kg)组,每组10只; 另取10只大鼠设为假手术组。各组分别1次/d灌胃给药(假手术组和模型组给予0.9%氯化钠水溶液),疗程4周。肉眼观察各组大鼠子宫内膜异位病灶外观并行盆腔粘连评分,通过卡尺测定病灶体积,HE染色法行子宫内膜病理学检查,通过透射电子显微镜观察子宫内膜细胞线粒体超微结构病变,ELISA法检测腹腔液中炎症因子(TNF-α、IL-1β、IL-6)和转化生长因子-β1(TGF-β1)含量,Western blot法检测子宫内膜组织缺氧诱导因子-1α(HIF-1α)、血管内皮生长因子(VEGF)蛋白表达。结果:与假手术组相比,模型组大鼠子宫内膜肉眼可见囊泡样异位病灶形成、内含淡黄色液体、新生血管形成、与盆腔粘连; 模型组子宫内膜异位病灶间质致密、血管明显增生、炎性细胞浸润等病理学改变,线粒体数量增多但形态、结构未见异常; 腹腔液TNF-α、IL-1β、IL-6、TGF-β1含量升高(P<0.01),子宫内膜组织HIF-1α、VEGF表达上调(P<0.01)。与模型组相比,大蒜素中、高剂量组和孕三烯酮组子宫内膜异位病灶明显减小,粘连评分和病灶体积降低(P<0.05或P<0.01),子宫内膜异位病灶上皮细胞脱落、间质疏松、新生血管明显减少,线粒体数量减少且呈现肿胀、脊断裂溶解、脊间隙增大等超微结构改变; TNF-α、IL-1β、IL-6、TGF-β1含量降低(P<0.05或P<0.01); HIF-1α、VEGF表达下调(P<0.01)。结论:大蒜素具有抑制EMs大鼠异位病灶生长的作用,其机制可能与下调HIF-1α表达,抑制异位病灶血管新生、诱导线粒体损伤有关。
Abstract:
Objective:To investigate the effects of Allicin on the ectopic lesions in rats with endometriosis(EMs).Methods:The EMs rat model was prepared by auto-internal membrane transplantation,and the 50 model rats were randomly divided into model group,gestrinone group(0.5 mg/kg,positive control group)and low(5 mg/kg),medium(10 mg/kg),high-dose(20 mg/kg)Allicin group,n=10.Another 10 rats were set as the sham operation group.The drugs were given once a day by intraperitoneal injection in each group for 4 weeks.The appearance of endometriosis lesions were observed by naked eye and the adhesion scores was detected,the lesion volume was measured by calipers,the endometrium pathological examination was examed by HE staining method,and the ultrastructural lesions of endometrial cells mitochondria was observed by transmission electron microscope,the content of inflammatory factors(TNF-α,IL-1β,IL-6)and TGF-β1 in peritoneal fluid were detected by ELISA method; the expression of HIF-1α and VEGF protein in endometrial tissue were detected by Western blot method.Results:Compared with the sham operation group,the endometrium of the rats in the model group showed vesicle-like ectopic lesion formation,containing light yellow liquid,neovascularization and pelvic adhesions; the interstitium of endometriotic lesions in the model group was dense,with pathological changes such as obvious vascular proliferation and inflammatory cell infiltration.the number of mitochondria had increased,but the morphology and structure were not abnormal.the content of TNF-α,IL-1β,IL-6 and TGF-β1 in the peritoneal fluid were increased(P<0.01).The expression of HIF-1α,VEGF in the endometrium tissue was up-regulated(P<0.01).Compared with the model group,the middle-,high-dose Allicin groups and the gestrinone group had significantly reduced endometriotic lesions,decreased dhesion score and lesion volume(P<0.05 or P<0.01); ectopic lesions a include epithelial cell shedding,interstitial porosity,neovascularization,and the mitochondria showed swelling,spinal fracture and dissolution,and spinal space enlargement and other ultrastructural lesions; the content of TNF-α,IL-1β,IL-6 and TGF-β1 were decreased(P<0.01),the expression of HIF-1α,VEGF were down-regulated(P<0.01).Conclusion:Allicin can inhibit the growth of ectopic lesions in EMs rats; which mechanism may be related to down-regulating the expression of HIF-1α,and then inhibiting angiogenesis in ectopic lesions and inducing mitochondrial damage.
