摘 要 目的：探讨慢性乙型肝炎（CHB）患者肠源性内毒素血症（TEM）对外周血Treg细胞、Th17细胞及对肝功能的影响。方法：收集35 例健康对照者（HC）和70例CHB患者且血浆内毒素阳性患者的外周血；应用流式细胞术测定外周血 Treg 细胞（CD4+Fox P3+）及 Th17 细胞（CD4+IL17）的水平；并结合血浆谷丙转氨酶（ALT）、谷草转氨酶（AST）和总胆红素（TBIL）进行分析。结果：与健康对照者比较，CHB 患者的Treg细胞百分比、Th17 细胞百分比显著升高（P＜0.05）；CHB内素素阳性组患者（对照组）Treg细胞百分比、Th17 细胞百分比水平均明显高于研究组（P＜0.05）。并且治疗后研究组肝功能指标血浆谷丙转氨酶（ALT）、谷草转氨酶（AST）和总胆红素（TBIL）的改善较对照组更为明显（P＜0.05）。两组患者不良反应发生率差异不具有统计学意义（P＞0.05）。结论：自拟祛毒退黄汤结肠滴注给药对慢性乙型肝炎肠源性内毒素血症症状和肝功能均有较为明显的改善作用，副作用小，值得临床应用。
Abstract:
Abstract Objective： To investigate the effects of intestinal endotoxemia (TEM) on peripheral blood Treg cells, Th17 cells and liver function in patients with chronic hepatitis B (CHB). Methods: Peripheral blood of 35 healthy controls (HC) and 70 patients with CHB and plasma endotoxinpositive patients were collected. Peripheral blood Treg cells (CD4+Fox P3+) and Th17 cells (CD4+IL17) were determined by flow cytometry+) level; combined with plasma alanine aminotransferase (ALT) aspartate aminotransferase (AST) and total bilirubin (TBIL) for analysis. Results：Compared with healthy controls, the percentage of Treg cells and the percentage of Th17 cells in CHB patients were significantly increased (P<0.05). The percentage of Treg cells and the percentage of Th17 cells in CHB endogenous positive group (control group) were significantly higher than those in the study(P<0.05). The improvement of plasma alanine aminotransferase (ALT), aspartate aminotransferase (AST) and total bilirubin (TBIL) in the liver function of the study group was more significant than that of the control group (P<0.05). There was no significant difference in the incidence of adverse reactions between the two groups (P>0.05). Conclusion:The colon injection of Qudu Tuihuang decoction can significantly improve the symptoms of intestinal endotoxemia and liver function of chronic hepatitis B. The side effects are small and worthy of clinical promotion.
