摘要:
摘 要 目的：探讨经穴针刺对老年脑卒中患者脑灌注及血小板反应蛋白1（THBS1）水平的影响。方法：选取94例脑卒中老年患者按单盲法分为两组，各47例。对照组采用常规西药治疗，在此基础上，试验组采用经穴针刺治疗，末次治疗结束后，比较两组临床疗效、脑灌注情况及THBS1水平、NIHSS评分、Barthel 指数、FCA、不良反应。结果：试验组总有效率93.62%较对照组74.47%为高，差异具有统计学意义（P＜005）。治疗后试验组PCA（36.98±4.16）cm/s、ACA（36.56±5.17）cm/s、MCA（48.19±6.02）cm/s的平均血流速度较对照组\[（32.05±3.62）cm/s、（33.84±5.02）cm/s、（43.02±5.37）cm/s\]高，THBS1水平（210.29±40.11）μg/L较对照组（253.26±38.62）低，差异具有统计学意义（P＜005）。治疗后试验组NIHSS评分（9.85±2.69）分较对照组（13.39±3.51）分低；BI指数（76.26±10.08）分、FCA评分（70.35±10.06）分较对照组（68.95±9.55）分、（60.35±9.51）分高，差异具有统计学意义（P＜005）。试验组不良反应率10.64%与对照组8.51%相比，差异无统计学意义（P＞005）。结论：经穴针刺治疗老年脑卒中效果较好，可改善脑灌注情况，促进患者神经功能、综合功能恢复，提升患者日常生活能力，且不良反应少，安全性高。
Abstract:
Abstract Objective:To investigate the effect of acupoint acupuncture on cerebral perfusion and thrombospondin 1 (THBS1) in senile patients with stroke. Methods：94 senile patients with stroke in the hospital were divided into two groups by single blind method，with 47 cases in each group. Control group took routine western medicine，and experimental group was treated with acupoint acupuncture based on control group. After last treatment，the clinical efficacy，cerebral perfusion mean flow velocity of PCA，ACA，MCA，THBS1，NIHSS，BI，FCA，adverse reactions were compared between two groups. Results：The total effective rate of experimental group 93.62% was higher than that of control group 74.47%(P＜005).After treatment，the mean flow velocity of PCA，ACA，MCA in experimental group \[(36.98±4.16)cm/s，(36.56±5.17)cm/s，(48.19±6.02)cm/s\] was higher than that in control group \[(32.05±3.62)cm/s，(33.84±5.02)cm/s，(43.02±5.37)cm/s\]，while the level of THBS1 in experimental group \[(210.29±40.11)μg/L\] was lower than that in control group (253.26±38.62)(P＜005). After treatment，the NIHSS score of experimental group (9.85±2.69)points was lower than that of control group (13.39±3.51)points，while BI and FCA scores in experimental group \[(76.26±10.08)points，(70.35±10.06)points\] were higher than those in control group \[(68.95±9.55)points，(60.35±9.51)points\](P＜005).There was no statistical difference in the incidence of adverse reactions between experimental group 10.64% and control group 8.51% (P＞005). Conclusion：Acupoint acupuncture in treatment of senile patients with stroke is effective，it can improve cerebral perfusion，promote recovery of nerve function and comprehensive function，increase activity of daily living，have less adverse reactions and high safety. 
