摘 要 目的：观察云南白药胶囊对老年股骨转子间骨折股骨近端防旋髓内针（PFNA）内固定术患者围手术期隐性失血、术后髋关节活动的影响。方法：选择72例老年股骨转子间骨折患者随机分为对照组和观察组各36例。患者均由同一组医生行PFNA内固定术，对照组患者术后给予常规西药治疗；观察组患者在对照组基础上给予云南白药胶囊口服。比较两组患者的围手术期失血量，术后评价骨折髋关节功能恢复情况，观察疼痛、肿胀的情况，记录两组患者的骨折愈合时间、下床活动时间、住院时间，统计并发症的发生率。结果：观察组患者围手术期失血量较对照组显著减少（ P ＜0.05）；术后髋关节功能活动优良率86.11%显著高于对照组的72.22% ( P ＜0.05)；术后疼痛评分、肿胀值改善更为明显 ( P ＜0.05)；〖JP2〗骨折愈合时间、下床活动时间、住院时间明显短于对照组（ P ＜0.05）。结论：云南白药胶囊利于减少老年股骨转子间骨折PFNA内固定术围手术期的失血，促进术后髋关节功能的恢复，且并发症少。
Abstract:
Abstract Objective：To investigate the effects of Yunnan Baiyao capsules on perioperative recessive blood loss and postoperative hip motion in elderly patients with proximal femoral nail antirotation(PFNA) internal fixation for intertrochanteric fracture of femur.Methods:72 elderly patients with intertrochanteric fracture of the femur were admitted and selected as objects，and were randomly divided into the control group and the observation group with 36 cases each.All the patients were treated by the same group of doctors with PFNA internal fixation.Patients in the control group were treated with conventional western medicine after surgery.Patients in the observation group were given Yunnan Baiyao capsules orally on the basis of the control group.The purpose of this study was to compare the perioperative blood loss of patients，evaluate the functional recovery of fracture hip joint after the operation，observe the pain and swelling before and after treatment，record the fracture healing time，out of bed activity time and hospital stay of the two groups，and count the incidence of complications.Results：Perioperative blood loss was significantly reduced in the observation group compared with the control group ( P ＜ 0.05).The optimal rate of functional activity of the hip joint was 86.11% in the observation group，which was significantly higher than 72.22% in the control group ( P ＜0.05).After surgery，the pain score and swelling value of the two groups were significantly lower than that of the preoperative group，and the pain score and swelling value of the observation group were significantly improved ( P ＜ 0.05).The time of fracture healing，the time of getting out of bed，and the time of hospitalization were significantly shorter than that of the control group ( P ＜0.05).Conclusion：Yunnan Baiyao capsule is beneficial to reduce perioperative blood loss in elderly patients with PFNA internal fixation for femoral intertrochanteric fracture，promote postoperative recovery of hip function，and reduce complications.〖LM〗
