摘 要 目的：探讨参丹散结胶囊联合经超声引导下的多针扇形交叉立体定位单极冷循环射频消融术治疗老年结直肠癌肝转移的临床疗效。方法：选取224例老年结直肠癌肝转移患者纳入本研究，并采用随机数字表法将其分为观察组与对照组，每组112例。对照组采用经超声引导下的多针扇形交叉立体定位单极冷循环射频消融术治疗，观察组在此基础上另给予参丹散结胶囊。治疗3个月后，评价两组临床疗效。治疗前后，分别取两组患者静脉血，检测其中肝功能指标含量\[总胆红素（TBIL）、碱性磷酸酶（ALP）、丙氨酸氨基转移酶（ALT）及天冬氨酸氨基转移酶（AST）\]，免疫功能指标含量（CD3+、CD4+、IgG、IgA及IgM）术后，观察两组患者的并发症情况（发热、术后疼痛、术后感染、出血、胸腔积液、黄疸及腹水等）。治疗后，对两组患者进行为期3年的随访工作，观察患者1年生存率、3年生存率，无疾病进展生存时间（PFS）和总生存时间（OS）。结果：治疗后，观察组客观缓解率及疾病控制率分别为67.9 %和87.7 %（P＜0.05）。治疗后，两组外周血TBIL、ALP、ALT及AST含量均明显降低，且观察组均明显低于对照组（P＜0.05）；CD3+、CD4+、IgG、IgA和IgM含量均明显增高，且观察组均明显高于对照组（P＜0.05）。治疗期间，观察组各项并发症发生率与对照组均基本一致（P＜0.05）。治疗后，观察组1年生存率、3年生存率、PFS和OS均明显高于对照组（P＜0.05）。结论：参丹散结胶囊联合经超声引导下的多针扇形交叉立体定位单极冷循环射频消融术治疗老年结直肠癌肝转移疗效较好，可有效改善患者免疫功能及肝功能，延长患者生存时间。
Abstract:
Abstract Objective：Investigate the clinical effects of Shendan Sanjie capsule combined with multi-needle sector cross-dimensional positioned monopole laparoscopic cool-tip radiofrequency ablation in the treatment of elderly liver metastases of colorectal cancer. Methods：224 elderly cases of liver metastases of colorectal cancer were randomly divided into observation group and control group,112 cases in each group. The control group was treated by multi-needle sector cross-dimensional positioned monopole laparoscopic cool-tip radiofrequency ablation and the observation group was given Shendan Sanjie capsule on the base of control group. The effects were evaluated in the two groups after 3 months treatment. The levels of liver function indicators (TBIL,ALP,ALT and AST),and immunological function indicators (CD3+,CD4+,IgG,IgA and IgM) were detected in the patients at before and after treatment. During the treatment,the complications in the two groups were observed. After the treatment,patients in two groups were followed up for 3 years. The 1 year survival rate,3 years survival rate,progress free survival (PFS) and overall survival (OS) were observed. Results：After the treatment,the objective remission rate and disease control rate in observation group were significantly higher than control group (P＜0.05). After the treatment,the levels of 〖LM〗TBIL,ALP,ALT and AST in the observation group were significantly lower than control group (P＜0.05),and CD3+,CD4+,IgG,IgA and IgM levels were significantly higher than control group (P＜0.05). During the treatment,the rates of complications in observation group were similar with the control group (P＞ 0.05). The long-term effects showed that 1 year survival rate,3 years survival rate,PFS and OS in observation group were significantly higher than control group (P＜0.05). Conclusion：Shendan Sanjie capsule combined with multi-needle sector cross-dimensional positioned monopole laparoscopic cool-tip radiofrequency ablation in the treatment of elderly liver metastases of colorectal cancer was effective,can promote the immune function and liver function,lengthen the survival time in the patients.
