摘要:
摘 要 目的:观察芪精丹兰汤对伴有左心室舒张功能不全(LVDD)的2型糖尿病(T2MD)合并代谢综合征(MS)患者糖脂代谢、心率变异性(HRV)的影响。方法:将95例伴有LVDD的T2MD合并MS患者按照随机数字表法分为观察组和对照组,每组各45例,对照组给予常规西医治疗,观察组在对照组基础上给予芪精丹兰汤治疗,两组疗程均为3个月。记录两组治疗前后糖脂代谢指标、HRV指标、心功能指标的变化。并统计两组临床疗效。结果:两组治疗后空腹血糖(FPG)、餐后2h血糖(2hPG)、糖化血红蛋白(HbA1c)、稳态模型胰岛素抵抗指数(HOMAIR)均较治疗前显著降低(P均<0.05),观察组治疗后以上指标均低于对照组(P均<0.05);两组治疗后甘油三酯(TG)、总胆固醇(TC)、低密度脂蛋白胆固醇(LDLC)、高密度脂蛋白胆固醇(HDLC)水平均较治疗前显著改善(P均<0.05),观察组治疗后以上指标改善情况均优于对照组(P均<0.05);两组治疗后RMSSD、SDANN、SDNN、PNN50%、LF、HF均显著升高(P均<0.05),LF/HF降低,观察组治疗后以上指标均优于对照组(P均<0.05);两组治疗后超声心动图检查指标e′、E/e′、E/A较治疗前显著改善(P均<0.05),观察组治疗后以上指标均优于对照组(P均<0.05);观察组临床治疗总有效率显著高于对照组(P均<0.05)。结论:在西医治疗基础上加用芪精丹兰汤可显著改善伴有LVDD的T2MD合并MS患者的糖脂代谢紊乱状态,减轻胰岛素抵抗,并改善HRV,有利于左心室舒张功能的恢复。
Abstract:
Abstract 〖HT5”SS〗Objective:To observe the effect of Qijing Danlan decoction on glucose and lipid metabolism and heart rate variability (HRV) in patients with type 2 diabetes mellitus (T2MD) complicated with metabolic syndrome (MS) with left ventricular diastolic dysfunction (LVDD). Methods:95 patients with T2MD combined with MS with LVDD were divided into observation group and control group according to the random number table method,45 patients in each group. The control group was given conventional western medicine treatment,and the observation group was given Qijing Danlan decoction treatment on the basis of the control group. The treatment course of both groups was 3 months. The changes of glucose and lipid metabolism index,HRV index and cardiac function index before and after treatment were recorded. The clinical effects of the two groups were statistically analyzed. Results:The fasting blood glucose (FPG),2h postprandial blood glucose (2hPG),glycosylated hemoglobin (HbA1c) and homeostasis model insulin resistance index (homair) of the two groups were significantly lower after treatment than before treatment (all P<0.05). The levels of triglyceride (TG),total cholesterol (TC),lowdensity lipoprotein cholesterol (ldlc) and highdensity lipoprotein cholesterol (hdlc) in the two groups were significantly improved after treatment (all P<0.05). The improvement of the above indicators in the observation group was better than that in 〖LM〗the control group (all P<0.05). RMSSD,SDANN,SDNN,PNN50%,LF and HF were significantly increased in the two groups after treatment (all P<0.05),while LF/HF was decreased. The above indicators in the observation group were better than those in the control group after treatment (all P<0.05). The echocardiographic indexes e ',e /e 'and e /A in the two groups were significantly improved after treatment (all P<0.05). The above indexes in the observation group were superior to those in the control group (all P<0.05). The total effective rate in the observation group was significantly higher than that in the control group (all P<0.05). Conclusion:on the basis of western medicine treatment,Qijing Danlan decoction can significantly improve the disorder of glucose and lipid metabolism in T2MD patients with MS complicated with LVDD,reduce insulin resistance,and improve HRV,which is conducive to the recovery of left ventricular diastolic function.
