摘要:
摘要目的：规范评价神经复元方治疗缺血性卒中后抑郁（PSD）的临床疗效及对血清BDNF的影响。方法：采用随机双盲对照的研究方法，将符合纳入标准的120例PSD患者分为治疗组和对照组。在缺血性卒中治疗基础上，治疗组予以神经复元方治疗，对照组予以中药安慰剂，疗程8周。治疗前后运用HAMD、MBI、ADL、NIHSS和中医症候评分量表评价患者的抑郁和神经功能变化情况。同时进行安全性评定和采用酶联免疫吸附试验检测比较两组血清BDNF含量。结果：治疗组HAMD、MBI、ADL、NIHSS和中医症候评分在治疗4周后明显优于对照组（P＜0.01）；治疗组血清BDNF含量显著高于对照组（P＜0.01）。结论：神经复元方可改善PSD患者抑郁状态和生活质量，一定程度上修复神经功能，疗效机制可能与上调BDNF有关。
Abstract:
AbstractObjective:To normatively evaluate the clinical efficacy of Shenjing Fuyuan decoction in treating poststroke depression and its effect on serum BDNF.Methods:120 PSD patients were divided into treatment group and control group with randomized doubleblind control method.On the basis of the treatment of ischemic stroke,the treatment group was treated with Shenjing Fuyuan decoction,while the control group was treated with Chinese medicine placebo for 8 weeks.HAMD,MBI,ADL,NIHSS and TCM Symptom rating scale were used to evaluate the depression and neurologic changes before and after treatment.In the meanwhile,the serum BDNF contents in the two groups were measured by ELISA and safety evaluation.Results:The scores of HAMD,MBI,ADL,NIHSS and TCM syndrome were significantly higher in the treatment group than those in the control group after 4 weeks of treatment (P＜0.01).The serum BDNF level in the treatment group was obviously higher than that in the control group (P＜0.01).Conclusion:Shenjing Fuyuan decoction can effectively improve the depression of PSD patients,and it can significantly improve the neurologic recovery and the ability of daily living of patients.The therapeutic mechanism may be related to upregulation of BDNF.
