摘要:
摘要目的：探讨狼疮性肾炎中医证型与临床病理分型的关系。方法：收集行肾脏病理活检并确诊为狼疮性肾炎患者57例，收集患者一般临床资料、实验室检查、肾脏病理分型，进行中医辨证分型，探讨中医证型与实验室指标及肾脏病理分型的关系。结果：57例狼疮性肾炎患者，中医证型以脾肾气虚型多见，病理分型以IV型常见，临床分型以肾炎综合征常见，狼疮活动度以轻度活动为主，系统性红斑狼疮疾病活动指数（SLEDAI）评分脾肾气虚型、气阴两虚型均低于其他几型，24 h尿蛋白定量脾肾气虚型最高，血白蛋白、IgG、补体C3、C4以脾肾气虚型最低，红细胞沉降率、超敏C反应蛋白以气阴两虚型最高，抗核抗体、抗Sm抗体阳性率均＞50%。结论：中医证型以脾肾气虚型最多见；临床分型以肾炎综合征最常见；病理分型以IV型最常见，而脾肾气虚型临床分型多表现为肾炎综合征，病理分型多表现为IV型；脾肾气虚型尿蛋白定量最大，且血白蛋白、ANA、抗Sm抗体、抗dsDNA、补体C3、C4降低阳性率均较高，为临床医师制定中医方案提供了参考，可以更好的指导治疗及改善预后。
Abstract:
AbstractObjective:To explore the relationship between TCM syndrome types and clinical pathological types of lupus nephritis.Methods: 57 patients with lupus nephritis who underwent renal pathological biopsy were collected.General clinical data,laboratory examinations and pathological types of kidney were collected.TCM syndrome differentiation and typing were carried out.The relationship between TCM syndrome types and laboratory indicators and pathological types of kidney was explored.Results: Among 57 patients with lupus nephritis,spleenkidney Qi deficiency type was the most common syndrome in TCM,type IV was the most common pathological type,nephritis syndrome was the most common clinical type,slight activity was the main activity of lupus,and activity integral of systemic lupus erythematosus disease (SLEDAI) was lower than other types in both spleenkidney Qi deficiency type and QiYin deficiency type,24 h urine protein quantification of spleen and kidney Qi deficiency type was the highest,serum albumin,IgG,complement C3,C4 spleen and kidney Qi deficiency type was the lowest,erythrocyte sedimentation rate and hypersensitive Creactive protein Qi and Yin deficiency type were the highest,and the positive rates of antinuclear antibody and antiSm antibody were all more than 50%.Conclusion: Spleenkidney Qi deficiency is the most common syndrome in TCM,nephritis syndrome is the most common clinical type,type IV is the most common pathological type,while type IV is the most common clinical type,and type IV is the most pathological type.Quantitative urinary protein in spleenkidney Qi deficiency type is the largest,and albumin,ANA,antiSm antibody and antids are the most common.The positive rates of DNA,complement C3 and C4 were higher,which provided a reference for clinicians to formulate TCM programs,and can better guide treatment and improve prognosis.
