摘要:
摘要目的：探讨益胃汤加减联合三联疗法对HP感染所致慢性胃炎患者血清炎性因子、HP清除率的影响。方法：选择112例HP感染的慢性胃炎患者作为分析对象，按照随机数字表法将其分为联合组与对照组各56例。对照组患者给予常规西药三联疗法治疗，联合组患者使用益胃汤加减联合三联疗法治疗。比较治疗前、治疗4周后两组患者中医证候（胃脘隐痛、嗳气反酸、纳呆食少、恶心呕吐、神疲乏力）积分、血清炎性因子\[表皮生长因子（EGF）、白细胞介素32（IL32）、转化生长因子β1（TGFβ1）、一氧化氮（NO）\]水平变化情况，记录两组患者临床有效率及HP转阴率。结果：治疗4周后，两组患者中医证候积分明显低于治疗前，且联合组低于对照组（P＜005）；联合组患者血清炎性因子EGF、IL32、TGFβ1均低于治疗前，且联合组降低程度明显低于对照组（P＜005）；两组患者NO水平均高于治疗前，且联合组患者高于对照组（P＜005）。联合组患者临床总有效率及HP清除率明显高于对照组（P＜005）。结论：益胃汤加减联合三联疗法治疗HP感染所致慢性胃炎疗效显著，能有效改善患者临床症状，减轻炎症损伤，提高HP清除率。
Abstract:
AbstractObjective:To investigate the effects of addition and subtraction of Yiwei decoction combined with triple therapy on serum inflammatory factors and HP clearance in patients with chronic gastritis caused by HP infection.Methods:112 patients with HPinfected chronic gastritis were enrolled in the study.They were divided into combined group(56 cases)and control group(56 cases)according to the random number table method.Control group was treated with conventional western medicine triple therapy,and combined group was given addition and subtraction of Yiwei decoction combined with triple therapy.The TCM syndromes(dull stomachache,belching and sour regurgitation,anorexia and reduced appetite,nausea and vomiting,fatigue and weakness)and serum inflammatory factors\[epidermal growth factor(EGF),interleukin32(IL32),transforming growth factorβ1(TGFβ1),nitric oxide(NO)\] were compared between the two groups before treatment and after 4 weeks of treatment.The clinical effective rate and HP negative conversion rate were recorded in the two groups.Results:After 4 weeks of treatment,the scores of TCM syndromes in the two groups were significantly lower than those before treatment,and the scores in combined group were lower than those in control group(all P＜005).The serum inflammatory factors EGF,IL32 and TGFβ1 in combined group were lower than those before treatment,and the decreases in combined group were significantly lower than those in control group(all P＜005).The NO level in the two groups was higher than that before treatment,and the level in combined group was higher than that in control group(all P＜005).The total clinical effective rate and HP clearance rate in combined group were significantly higher than those in control group(all P＜005).Conclusion :Yiwei decoction combined with triple therapy has significant efficacy in treating chronic gastritis caused by HP infection,and it can effectively improve clinical symptoms,reduce inflammatory damage and improve HP clearance rate.
