摘要:
目的:观察护肝利水汤联合特利加压素对肝硬化腹水患者临床疗效、血清指标与门静脉压力指标的影响。方法:选取94例肝硬化腹水患者纳入本次研究,分别纳入西药组和联合组,西药组给予常规西药治疗,联合组在西药治疗方法的基础上加用自拟护肝利水汤; 1个疗程后,对比临床疗效,观察两组患者治疗前后临床与血清指标、门静脉压力指标及脾静脉内径的变化情况,统计治疗期间不良反应发生率。结果:两组临床疗效对比联合组更优(P<0.05),总有效率为联合组97.87%、西药组85.11%,差异均有统计学意义; 治疗后,两组患者24 h尿量、腹水深度、体重、腹围,Cre、ALB、PT,门静脉主干内径、门静脉血流量、门静脉血流速及脾静脉内径均较本组治疗前改善,且两组对比联合组更优(P<0.05)。结论:护肝利水汤联合特利加压素能够提高肝硬化腹水的临床疗效,进一步改善患者临床指标、血清指标与门静脉压力指标。
Abstract:
Objective:To observe the effect of Hugan Lishui decoction combined with terlipressin on clinical efficacy,serum indexes and portal pressure indexes of patients with liver cirrhosis and ascites.Methods:The objects of study were 94 patients with liver cirrhosis and ascites.According to the simple random method,they were divided into the western medicine group and the combined group.Western medicine group was given conventional western medicine treatment.On the basis of western medicine treatment method,combined treatment group was treated with Hugan Lishui decoction.After 1 course of treatment,the clinical effects were compared,and the changes in clinical and serum indicators,portal venous pressure,and splenic vein diameter were observed before and after treatment in the two groups.The incidence of adverse reactions was counted.Results:The clinical efficacy of the two groups was better than that of the combined group(P<0.05).The total effective rate was 97.87% in the combined group and 85.11% in the western medicine group,with statistically significant differences.After treatment,the urine volume,ascites depth,body weight,abdominal circumference,Cre,ALB,PT,internal diameter of main portal vein,portal venous blood flow,portal venous blood velocity and internal diameter of splenic vein of the two groups were improved after treatment compared with the two groups,and the two groups were better than the combined group(P<0.05).Conclusion:Hugan Lishui decoction combined with terlipressin can improve the clinical efficacy of liver cirrhosis and ascites,and further improve the clinical indicators,serum indexes and portal pressure indexes of patients.
