摘要:
目的:研究香砂六君子汤用于慢性萎缩性胃炎治疗的安全性及有效性。方法:选入慢性萎缩性胃炎患者合并HP感染80例,80例患者分为两组,西药治疗组40例(标准四联杀菌方案),香砂六君子汤组40例(香砂六君子汤辨证给药),对比测评两组临床疗效、胃镜疗效、病理疗效(胃黏膜萎缩、肠上皮化生、异型增生),记录患者治疗期间毒副反应发生情况,检测两组幽门螺旋杆菌(HP)转阴情况,随访6个月,记录复发情况。结果:胃镜疗效观察中香砂六君子汤组治疗有效率显著更高(P<0.05),黏膜萎缩、肠上皮化生均为香砂六君子汤组治疗有效率更高(P<0.05),两组异型增生治疗有效率对比,差异无统计学意义(P>0.05),临床疗效评估提示香砂六君子汤组临床有效率显著更高(P<0.05),HP转阴情况观察中香砂六君子汤组转阴率显著更高(P<0.05),香砂六君子汤组不良反应发生率、近期复发率明显低于西药治疗组,差异存在统计学意义(P<0.05)。结论:香砂六君子汤用于慢性萎缩性胃炎的治疗中具有改善炎症反应、缓解临床症状、抑制杀灭HP等多种效果。
Abstract:
Objective:To study the safety and effectiveness of Xiangsha Liujunzi decoction for the treatment of chronic atrophic gastritis.Methods:80 cases of chronic atrophic gastritis patients with HP infection were selected.The 80 patients were divided into two groups,40 cases were treated with western medicine(standard quadruple sterilization scheme),and Xiangsha Liujunzi decoction group 40 cases(Xiangsha Liujunzi decoction),compared and evaluated the clinical effect,gastroscopy effect,pathological effect(gastric mucosal atrophy,intestinal metaplasia,dysplasia)of the two groups,and recorded the occurrence of toxic and side effects during the treatment.The H.pylori(HP)turned negative was detected in two groups,followed up for 6 months,and the recurrence was recorded.Results:The efficacy of gastroscopy in the treatment of Xiangsha Liujunzi decoction group was significantly higher(P<0.05).Mucosal atrophy and intestinal metaplasia were all more effective in the Xiangsha Liujunzi decoction group(P<0.05).There was no significant difference in the effective rate of treatment of dysplasia between the two groups(P>0.05).The clinical efficacy evaluation showed that the clinical effectiveness of the Xiangsha Liujunzi decoction group was significantly higher(P<0.05).The negative rate was significantly higher in the group(P<0.05).The incidence of toxic and side effects and the recent recurrence rate in the Xiangsha Liujunzi decoction group were significantly lower than those in the western medicine treatment group,and the differences were statistically significant(P<0.05).Conclusion:Xiangsha Liujunzi decoction has various effects in the treatment of chronic atrophic gastritis,such as improving inflammatory response,alleviating clinical symptoms,and inhibiting and killing HP.
