摘要:
目的:观察骨前康颗粒与加味补中益气汤辅助防旋股骨近端髓内钉(PFNA)对老年股骨粗隆间骨折的治疗效果。方法:选取老年股骨粗隆间骨折患者为观察对象,根据其治疗方式分为对照组和观察组,两组患者均接受PFNA治疗,对照组在术后给予常规西药治疗,观察组在此基础上给予骨前康颗粒与加味补中益气汤治疗。观察两组患者愈合时间,比较两组治疗前后炎症因子水平、功能恢复和并发症发生率的差异。结果:观察组患者下床活动时间和骨折愈合时间分别为(42.35±6.01)d、(3.35±0.97)月,均较对照组短(t=17.982、7.517,P<0.001); 两组患者治疗前Harris评分无差别,治疗后,观察组患者Harris评分优于对照组(χ2=8.635,P=0.035); 观察组CRP、IL-6和TNF-α水平低于对照组(t=7.583、5.456、13.664,P<0.001); 两组患者畸形愈合、伤口感染、骨不连和髋内翻发生率无差别(χ2=0.346,P=0.556)。结论:骨前康颗粒与加味补中益气汤辅助PFNA可促进老年股骨粗隆间骨折患者的术后恢复。
Abstract:
Objective:To observe the therapeutic effect of Guqiankang granule and Jiawei Buzhong Yiqi decoction(PFNA)in the treatment of intertrochanteric fracture of femur in the aged.Methods:Senile femoral intertrochanteric fracture patient for treatment as the research object,according to the treatment methods are divided into control group and observation group,two patients were treated with PFNA,the control group was given after conventional western medicine treatment,the observation group was given Guqiankang granule and Jiawei Buzhong Yiqi decoction.The healing time of the two groups was observed,and the differences in the levels of inflammatory factors,functional recovery and the incidence of complications before and after treatment were compared between the two groups.Results:The time of bed activity and fracture healing time of the patients were(42.35±6.01)d and(3.35±0.97)m in the observation group were shorter than those of the control group(t=17.982,7.517,P<0.001).Two groups of patients before treatment Harris score no difference after treatment,Harris scores in observation group were better than the control group(χ2 =8.635,P=0.035); two groups of patients had no difference between the levels of proinflammatory cytokines,after treatment,the observation group were CRP,IL-6 and TNF- levels lower than the control group(t=7.583,5.456,13.664,P<0.001).Two groups of patients with wound infection,malunion,nonunion and hip varus rate difference(χ2 =0.346,P=0.556).Conclusion:Guqiankang granule and Jiawei Buzhong Yiqi decoction have a better therapeutic effect on elderly patients with intertrochanteric fractures of the femur,and can significantly promote the recovery of patients after operation.
