摘要:
目的:探讨贞芪扶正颗粒联合顺铂方案在中晚期宫颈癌术后的应用价值及对患者T淋巴细胞亚群的影响。方法:选择82例中晚期宫颈癌患者,按随机数字表法分为对照组与观察组,各41例,两组术后均应用多西他赛加顺铂(TP)化疗方案,观察组加用贞芪扶正颗粒治疗,比较两组疗效、不良反应、治疗前后患者T淋巴细胞亚群(CD3+、CD4+、CD8+、NK)、免疫功能(IgA、IgG、IgM)的变化,统计随访1年局部复发及远处转移率。结果:观察组总缓解率78.05%,较对照组56.10%高(P<0.05)。观察组Ⅰ～Ⅱ级贫血17.07%、胃肠道反应12.20%、白细胞降低12.20%、脱发21.95%,发生率均低于对照组(36.59%、39.02%、34.15%、43.90%)(P<0.05),其Ⅲ～Ⅳ级白细胞降低(0)、脱发发生率(0)低于对照组(12.20%、9.76%)(P<0.05)。治疗后,观察组CD3+、CD4+、NK细胞所占比例及IgA、IgG、IgM均上升,CD8+细胞所占比例降低,CD4+/CD8+比值上升,变化幅度高于同组治疗前与对照组治疗后(P<0.05)。两组随访复发、转移率比较差异无统计学意义(P>0.05)。结论:贞芪扶正颗粒联合TP方案用于中晚期宫颈癌术后有明显增效减毒效应,可优化疗效,降低不良反应,改善机体免疫功能。
Abstract:
Objective:To investigate the application value of Zhenqi Fuzheng granules combined with cisplatin in middle and advanced stage cervical cancer after surgery and the influence on T lymphocyte subsets.Methods:82 patients with middle and advanced stage cervical cancer were enrolled,and they were divided into control group and observation group according to the random number table method,with 41 cases in each group.Both groups were given docetaxel with cisplatin(TP)chemotherapy.The observation group was additionally given Zhenqi Fuzheng granules.The curative effect,toxic effects,changes in T lymphocyte subsets(CD3+,CD4+,CD8+,NK)and immune function(IgA、IgG、IgM)before and after treatment were compared between the two groups.The local recurrence and distant metastasis rates were statistically analyzed after 1 year of followed up.Results:The total remission rate was higher in the observation group than that in the control group(78.05% vs 56.10%)(P<0.05).The incidence rates of I to II anemia(17.07%),gastrointestinal reaction(12.20%),leukopenia(12.20%)and alopecia(21.95%)in the observation group were lower than those in the control group(36.59%,39.02%,34.15%,43.90%)(P<0.05).The incidence rates of III to IV leukopenia(0)and alopecia(0)were lower than those in the control group(12.20%,9.76%)(P<0.05).After treatment,the proportions of CD3+,CD4+ and NK cells,IgA,IgG,IgM and the ratio of CD4+ to CD8+ in the observation group were increased,while the proportion of CD8+ cells was decreased.Changes were more obvious than those in the same group before treatment and in the control group after treatment(P<0.05).There was no significant difference in follow-up recurrence rate or metastasis rate between the two groups(P>0.05).Conclusion:There is obvious effect-enhancing and toxicity-reducing effect for the application of Zhenqi Fuzheng granules with cisplatin in middle and advanced stage cervical cancer after surgery,which can optimize the curative effect,reduce the toxic effects,and improve the immune function.
