摘要:
目的:观察益气活血通络汤联合阿司匹林预防人工髋关节置换(AHR)术后深静脉血栓(DVT)形成的临床疗效。方法:将154例AHR术后患者随机分为三组:中药组53例,采用益气活血通络汤联合阿司匹林治疗; 对照组A 51例,采用阿司匹林治疗; 对照组B 50例,采用低分子肝素钠(LMWH)治疗。比较三组患者围手术期血红蛋白值(Hb)、D-二聚体水平、下肢DVT发生率及术后切口并发症,下肢肿胀程度发生情况,评价预防疗效。结果:术后14 d,中药组血红蛋白高于其他两组(P<0.05),对照组A高于对照组B(P<0.05),D-二聚体水平低于其他两组(P<0.05),对照组A与对照组B比较,差异无统计学意义(P>0.05)。术后30 d,中药组DVT发生率低于其他两组(P<0.05),对照组A与对照组B比较,差异无统计学意义(P>0.05),有症状DVT 发生率三组间比较,差异无统计学意义(P>0.05)。中药组术后切口并发症发生率低于其他两组(P<0.05),对照组A与对照组B比较,差异无统计学意义(P>0.05)。中药组术后下肢肿胀低于其他两组(P<0.05),对照组A与对照组B比较,差异无统计学意义(P>0.05)。结论:益气活血通络汤联合阿司匹林能有效降低患者AHR术后深静脉血栓形成风险,减少切口并发症发生率,减轻下肢肿胀。
Abstract:
Objective:To observe the clinical efficacy of Yiqi Huoxue Tongluo decoction combined with aspirin in preventing deep vein thrombosis(DVT)after artificial hip replacement(AHR).Methods:Randomly divide 154 patients after AHR into three groups:Chinese medicine group,53 cases were treated with Yiqi Huoxue Tongluo decoction combined with aspirin; control group A,51 cases were treated with aspirin; control group B,50 cases were treated with low molecular weight heparin sodium(LMWH)treatment.Results:After 14 days,the hemoglobin of the Chinese medicine group was higher than that of the other two groups(P<0.05),the control group A was higher than the control group B(P<0.05),and the level of D-dimer was lower than the other two groups(P<0.05).At 30 days after surgery,the incidence of DVT in the Chinese medicine group was lower than that of the other two groups(P<0.05).There was no statistically significant difference between the control group A and the control group B(P>0.05).Academic significance(P>0.05).The incidence of postoperative incision complications in the Chinese medicine group was lower than that in the other two groups(P<0.05).There was no significant difference between the control group A and the control group B(P>0.05).The postoperative lower limb swelling of the Chinese medicine group was lower than that of the other two groups(P<0.05).There was no significant difference between the control group A and the control group B(P>0.05).Conclusion:Yiqi Huoxue Tongluo decoction combined with aspirin can effectively reduce the risk of DVT after AHR,reduce the incidence of incision complications,and reduce the swelling of lower extremities.
