摘要:
目的:探讨中药芒硝联合化石利胆汤对腹腔镜胆总管切开取石术后患者炎性因子的影响。方法:选择腹腔镜胆总管切开取石术患者112例作为对象,按随机数字表分为对照组和观察组各56例。对照组术后采用常规方法干预,观察组在对照组基础上术后即刻采用中药芒硝联合化石利胆汤治疗,术后对患者进行6个月随访,比较两组肝功能水平、炎性因子水平、复发率及不良反应发生率。结果:术后5 d,两组患者 DBiL水平比较差异无统计学意义(P>0.05); 观察组术后5 d,患者 AST、ALT及TBiL水平,均低于对照组(P<0.05); 观察组术后5 d 炎症因子IL-1、IL-6及CRP水平,均低于对照组(P<0.05); 观察组术后3个月、6个月复发率,均低于对照组(P<0.05); 两组术后电解质紊乱、内毒素血症、胃肠功能恢复延迟发生率比较差异无统计学意义(P>0.05)。结论:将中药芒硝联合化石利胆汤用于腹腔镜胆总管切开取石术后康复中有助于改善患者肝功能水平,降低炎性因子水平、复发率,并未增加不良反应发生率。
Abstract:
Objective:To explore the application of traditional Chinese medicine Glauber's salt combined with Huashi Lidan decoction in the rehabilitation of laparoscopic common bile duct stone removal and its effect on the level of inflammatory factors.Methods:A total of 112 patients with laparoscopic common bile duct stone lithotomy were enrolled.The random number table was divided into control group(n=56)and observation group(n=56).The control group was treated with routine methods after operation.The observation group was treated with Glauber's salt combined with Huashi Lidan decoction.The patients were followed up for 6 months.Results:There was no significant difference in DBiL levels between the two groups(P>0.05).The levels of AST,ALT and TBiL in the observation group were lower than those in the control group(P<0.05).The levels of IL-1 and CRP were lower than those of the control group(P<0.05).The recurrence rate of the observation group at 3 months and 6 months was lower than that of the control group(P<0.05).There was no significant difference in the incidence of electrolyte imbalance,endotoxemia and gastrointestinal function recovery(P>0.05).Conclusion:The use of traditional Chinese medicine Glauber's salt combined with Huashi Lidan decoction in the rehabilitation of laparoscopic common bile duct incision can help improve liver function,reduce the level of inflammatory factors and recurrence rate,and increase the incidence of adverse reactions.
