摘要:
目的:观察行健汤联合穴位贴敷减轻进展期胃癌患者癌因性疲乏的疗效,及对患者T细胞亚群、Th1/Th2细胞因子的调节。方法:将62例胃癌患者随机分为对照组和治疗组。两组患者均接受SOX方案化疗,治疗组加服行健汤,并配合穴位贴敷。用癌因性疲乏量表(Piper量表)评价患者治疗前后疲劳程度; 采用流式细胞仪检测外周血 T 细胞亚群、NK 细胞的分布比率,酶联免疫吸附(ELISA)法检测IL-2、IL-4、IL-6、IL-10、IL-17A、肿瘤坏死因子、IFN-γ水平。 结果:治疗前,两组患者Piper评分比较无统计学差异(P>0.05)。经治,在改善Piper量表中的行为疲乏和躯体疲乏方面,治疗组改善效果优于对照组(P<0.05)。治疗组的轻度癌因性疲乏例数增多,重度例数明显减少,两组治疗后比较差异有统计学意义(P<0.05)。治疗前,两组患者T细胞亚群、Th1/Th2因子水平比较无统计学差异(P>0.05)。治疗后,治疗组可提高 CD4+、NK细胞、CD4+/CD8+比例,降低CD8+细胞比例(P<0.05),且效果优于对照组(P<0.05)。治疗组可降低IL-6、IL-17A、IL-10、IL-4,升高IFN-γ、TNF(P<0.05),且治疗组的效果优于对照组(P<0.05),在IL-2方面两组治疗前后比较无统计学差异(P>0.05)。 结论:行健汤联合穴位贴敷能有效降低进展期胃癌患者疲劳程度、提高患者生活质量,其作用机制与改善患者免疫功能、改善Th1/Th2漂移有关。
