摘要:
目的:探讨益肾补骨汤联合经皮后凸成形术(PKP)治疗骨质疏松性椎体压缩骨折(OVCF)的临床疗效。方法:将114例OVCF患者随机分为观察组与对照组各57例。对照组施行经PKP治疗,观察组在此基础上给予益肾补骨汤治疗。比较两组临床疗效、骨愈合情况(骨痂生长情况),观察治疗前、治疗2周后两组伤锥情况(Cobb角、伤椎前缘高度比)、腰部功能恢复情况[Oswestry 功能障碍指数(ODI)]、骨代谢情况[骨钙素(BGP)、骨密度(BMD)、碱性磷酸酶(ALP)]差异。结果:治疗2周后,观察组临床总有效率明显高于对照组(P<0.05); 观察组Cobb角较治疗前及对照组均明显减小(P<0.05); 观察组伤椎前缘高度比较治疗前急对照组显著升高(P<0.05); 观察组ODI评分较治疗前及对照组明显降低(P<0.05); 两组BGP、BMD及ALP均高于治疗前,且观察组高于同期对照组(P<0.05)。结论:益肾补骨汤联合PKP手术治疗OVCF患者疗效显著,利于恢复椎体高度并纠正后凸畸形,恢复患者腰椎功能,改善骨代谢状态。
Abstract:
Objective:To explore the clinical efficacy of Yishen Bugu decoction combined with PKP surgery on osteoporotic vertebral compression fractures(OVCF).Methods:114 cases of OVCF patients were randomly divided into observation group(n=57)and control group(n=57).Control group was treated with percutaneous kyphoplasty(PKP),and observation group was treated with Yishen Bugu decoction on this basis.The clinical efficacy and bone healing(bone callus growth)were compared between the two groups,and the injured vertebrae situation(Cobb angle,anterior height ratio of injured vertebrae),lumbar function recovery [Oswestry Dysfunction Index(ODI)] and bone metabolism [bone gla protein(BGP),bone mineral density(BMD),alkaline phosphatase(ALP)] were observed in the two groups before treatment and after 2 weeks of treatment.Results:After 2 weeks of treatment,the total clinical effective rate in observation group was significantly higher than that in control group(P<0.05).The Cobb angle in observation group was significantly reduced compared with that before treatment and that in control group(P<0.05).The anterior height ratio of injured vertebrae in observation group was significantly increased compared with that before treatment and that in control group(P<0.05).The ODI score in observation group was significantly reduced compared with that before treatment and that in control group(P<0.05).The BGP,BMD and ALP in the two groups were higher than those before treatment,and the indexes in observation group were higher than those in control group(P<0.05).Conclusion:Yishen Bugu decoction combined with PKP surgery has a significant efficacy on patients with OVCF,and it is conducive to restoring vertebral height,correcting kyphosis,restoring lumbar function and improving bone metabolism.
