摘要:
目的:研究降逆胃舒汤对幽门螺杆菌感染致萎缩性胃炎病理评分的功效。方法:选取238例幽门螺杆菌感染致萎缩性胃炎患者作为本次研究观察对象,入选患者遵循随机数字表法平均分配入对照组与治疗组,对照组以常规西药四联疗法治疗,治疗组在对照组疗法的基础上给予降逆胃舒汤治疗; 治疗3个月后,观察两组患者HP清除率并随访6个月统计复发率,观察两组患者中医证候积分、胃黏膜胃镜征象积分、胃黏膜病理评分及血清胃泌素-17(G-17)、血清胃蛋白酶原Ⅰ、Ⅱ(PGⅠ、PGⅡ)、PGⅠ/PGⅡ比值(PGR)治疗前、后的变化情况。结果:治疗前两组患者各项中医证候、胃黏膜胃镜积分、病理评分及血清指标表达水平比较,差异无统计学意义(P>0.05); 治疗后两组患者各项中医证候、胃黏膜胃镜积分、病理评分及血清指标表达水平比较,差异有统计学意义(P<0.05),治疗组均优于对照组(P<0.05); 治疗后3个月时治疗组HP清除率为88.24%、对照组为73.11%,随访6个月治疗组复发率为3.81%,对照组为18.39%,两组HP根除率、复发率比较,差异有统计学意义(P<0.05)。结论:降逆胃舒汤能够提高幽门螺杆菌清除率并降低复发,更有效缓解幽门螺杆菌感染致萎缩性胃炎的各项中医证候,进一步改善患者胃黏膜病理评分与胃镜征象,更良好的调节患者血清指标。
Abstract:
Objective:To study the effect of Jiangni Weishu decoction on the pathological score of atrophic gastritis caused by helicobacter pylori infection.Methods:The objects of study were 238 patients with atrophic gastritis caused by HP infection.Enrolled patients were randomly assigned to the control group and the treatment group.Patients in the control group were treated by quadruple therapy with conventional western medicine,and those in the treatment group were provided with Jiangni Weishu decoction on the basis of treatment in the control group.After 3 months of treatment,the HP clearance rate of the two groups was observed and patients were followed up for 6 months to count the recurrence rate.Further observation was focused on the changes of traditional Chinese medicine(TCM)syndrome integral,gastric mucosa gastroscope signs integral,gastric mucosa pathological score,as well as serum Gastrin-17(G-17),serum Pepsinogen Ⅰ and Ⅱ(PGⅠ,PGⅡ),and PGⅠ/PGⅡ ratio(PGR)before and after treatment.Results:There were no statistically significant differences in TCM syndrome integral,gastric mucosa gastroscope signs integral,pathological score and expression levels of related indexes in the serum between the two groups before treatment(P>0.05).After treatment,there were improvements in TCM syndrome integral,gastric mucosa gastroscope signs integral,pathological score and expression levels of related indexes in the serum in patients of the two groups(P<0.05),and the treatment group was superior to the control group(P<0.05).At 3 months after treatment,the HP clearance rate was 88.24% in the treatment group and 73.11% in the control group; besides,the recurrence rate was 3.81% in the treatment group and 18.39% in the control group after follow-up for 6 months.The clearance rate of HP in the treatment group was higher than that in the control group,and the recurrence rate was lower than that in the control group(P<0.05).Conclusion:Jiangni Weishu decoction can improve the HP clearance rate and reduce the recurrence,more effectively relieve various TCM syndromes,further improve the gastric mucosa pathological score and gastroscope signs,and better regulate the serum indexes of patients with atrophic gastritis caused by HP infection.
