摘要:
目的:探讨基于月经周期的中药汤剂对多囊卵巢综合征(PCOS)患者性激素水平、妊娠结局的影响。方法:将160例PCOS患者随机分为对照组(n=80)和观察组(n=80),对照组给予枸橼酸氯米芬片、炔雌醇环丙孕酮片常规西医治疗,观察组在此基础上基于月经周期中药汤剂治疗,比较两组患者临床症状变化、治疗效果、性激素水平、妊娠结局。结果:两组患者治疗前月经周期、经量、神疲乏力、腰膝酸软、经行腹痛、脉沉涩中医症候积分比较,差异无统计学意义(P>0.05),治疗后观察组患者中医症候积分明显低于对照组(P<0.05); 观察组治疗总有效率91.25%,对照组治疗总有效率77.50%,两组比较差异有统计学意义(P<0.05); 治疗前两组患者的黄体生成素(LH)、促卵泡激素(FSH)、睾酮(T)、雌二醇(E2)水平比较无差异统计学意义(P>0.05),治疗后观察组LH、T、E2水平明显低于对照组,FSH水平明显高于对照组(P<0.05); 观察组患者的月经正常率、排卵率、妊娠成功率均明显高于对照组,早期流产率明显低于对照组(P<0.05); 两组患者治疗期间并无不良反应发生。结论:基于月经周期的中药汤剂治疗PCOS,能改善患者的临床症状,促进性激素水平恢复正常,有助于排卵和妊娠周期恢复正常,提高妊娠率。
Abstract:
Objective:To explore the effects of traditional Chinese medicine(TCM)decoction based on menstrual cycle on levels of sex hormones and pregnancy outcomes in patients with polycystic ovary syndrome(PCOS).Methods:160 patients with PCOS were divided into control group and observation group according to the treatment method,80 cases in each group.The control group was treated with routine western medicine,while observation group was treated with TCM decoction based on menstrual cycle on basis of control group.The changes of clinical symptoms,curative effect levels of sex hormones and pregnancy outcomes were compared between the two groups.Results:Before treatment,there were no significant differences in menstrual cycle,menstrual blood volume or scores of TCM syndromes such as fatigue,soreness and weakness of waist and knees,abdominal pain and sunken pulse between the two groups(P>0.05).After treatment,the above scores of the observation group were significantly lower than the control group(P<0.05).The total effective rate of treatment in observation group [73/160(91.25%)] was significantly higher than control group [62/160(77.50%)](P<0.05).Before treatment,there were no significant differences in luteinizing hormone(LH),follicle stimulating hormone(FSH),testosterone(T)or estradiol(E2)levels between the two groups(P>0.05).After treatment,levels of LH,T and E2 in observation group were significantly lower than those in control group,while FSH level was significantly higher than that in the control group(P<0.05).The normal rate of menstrual cycle,ovulation rate and pregnancy success rate in observation group were significantly higher than those in control group,while early abortion rate was significantly lower than that in the control group(P<0.05).There were no adverse reactions during treatment in either group.Conclusion:TCM decoction based on menstrual cycle can significantly improve clinical symptoms,levels of sex hormones and curative effect on PCOS patients,which is conductive to recovery of ovulation and pregnancy cycle,and increasing pregnancy rate.
