摘要:
目的:探讨清热活血方联合阿奇霉素治疗儿童肺炎支原体肺炎(MPP)的临床疗效。方法:将134例患儿按随机数字表法分为对照组及治疗组,对照组67例给予阿奇霉素及对症支持治疗,治疗组67例在对照组基础上给予清热活血方治疗。对比分析两组患儿临床疗效及治疗前后血清炎性因子白介素-6(IL-6)、白介素-4(IL-4)、白介素-8(IL-8)及肿瘤坏死因子-α(TNF-α)水平、临床症状及肺部体征消失时间、C反应蛋白(CRP)、血沉(ESR)、白细胞计数、肺炎支原体(MP)血清学试验改善情况。结果:治疗组总有效率为89.55%,优于对照组的74.63%,差异具有统计学意义(P<0.05); 治疗组住院时间、临床症状及肺部体征消失时间与对照组相比,差异具有统计学意义(均P<0.05); 治疗后,两组血清炎性因子IL-6、IL-4、IL-8及TNF-α、CRP、ESR及MP血清学试验与治疗前相比均下降,且治疗组低于对照组(P<0.05); 但在治疗前后白细胞计数差异无统计学意义(P>0.05)。结论:清热活血方联合阿奇霉素治疗儿童MPP疗效显著,其机制可能与降低炎性因子水平,改善炎性指标有关。
Abstract:
Objective:To investigate the clinical efficacy of Qingre Huoxue decoction combined with azithromycin in the treatment of mycoplasma pneumonia in children.Methods:The 134 children were divided into control group and treatment group by random number table method.67 children in the control group were given azithromycin and symptomatic support treatment,and 67 children in the treatment group were given Qingre Huoxue decoction on the basis of the control group.The clinical efficacy,serum levels of inflammatory factors IL-6,IL-4,IL-8 and TNF-α,disappearance time of clinical symptoms and pulmonary signs,C-reactive protein,erythrocyte sedimentation rate,white blood cell count,and improvement of mycoplasma pneumoniae serology before and after treatment in the two groups were compared and analyzed.Results:The total effective rate in the treatment group was 89.55%,which was better than 74.63% in the control group,and the difference was statistically significant(P<0.05).Compared with the control group,the hospitalization time,clinical symptoms and disappearance time of pulmonary signs in the treatment group were statistically significant(all P<0.05).After treatment,serum inflammatory factors IL-6,IL-4,IL-8,TNF-α,C-reactive protein,erythrocyte sedimentation rate and mycoplasma pneumoniae in both groups decreased compared with before treatment,and the treatment group was lower than the control group(P<0.05).However,there was no statistically significant difference in WBC count before and after treatment(P>0.05).Conclusion:Qingre Huoxue decoction combined with azithromycin has a significant effect on mycoplasma pneumoniae in children,and its mechanism may be related to lowering the levels of inflammatory factors,and improving the inflammatory indicators.
