摘要:
目的:研究针刀疗法对肩周炎兔肩关节囊中白细胞介素6(IL-6)、高迁移率族蛋白1(HLGB1)、酸敏感离子通道蛋白3(ASIC3)蛋白和基因表达的影响。方法:健康雄性新西兰大白兔30只,按照随机数字表法分为空白组、模型组、针刀组,每组各10只。采用机械加冰敷制造肩周炎模型。密切关注造模前后各组肢体功能活动变化; 采用HE染色观察局部组织形态学变化; 采用Western blot法检测局部IL-6、HLGB1、ASIC3蛋白水平; 采用RT-PCR技术检测各组关节囊滑膜中IL-6、HLGB1、ASIC3的mRNA表达水平。结果:针刀松解法能改善局部病理状态,减少滑膜和肌间质纤维素渗出、粘连和机化,促进组织的修复; 与空白组比较,模型组兔肩关节滑膜IL-6、HLGB1、ASIC3基因和蛋白表达水平显著增加(均P<0.05); 与模型组相比,针刀组兔肩关节滑膜IL-6、HLGB1、ASIC3基因和蛋白表达水平显著降低(均P<0.05)。模型组IL-6、HLGB1、ASIC3蛋白含量显著增多,基因表达水平明显上调,与空白组相比有统计学差异(均P<0.05); 针刀组上述指标蛋白含量显著降低,且对上述基因表达水平具有明显抑制作用,与模型组、空白组相比有统计学差异(均P<0.05)。结论:针刀疗法可降低肩关节囊滑膜中IL-6、HLGB1、ASIC3蛋白表达水平,并能抑制其基因过度表达,这可能是针刀治疗肩周炎有效作用机制之一。
Abstract:
Objective:To study the effects of acupotomology on the expression of IL-6,HLGB1,ASIC3 and genes in periarthritis humeral rabbits.Methods:30 healthy male New Zealand white rabbits.Divided into blank,model,and acupotomy groups according to the random number table method,with 10 pieces in each group.Fabrication of a frozen shoulder model using mechanical plus ice application.Pay close attention to the changes in functional activity of each group of limbs before and after modeling,local histomorphological changes were observed using HE staining,detection of local IL-6,HLGB1 and ASIC3 protein levels by Western blot method.The mRNA expression levels of IL-6,HLGB1 and ASIC3 in synovium of joint capsule were detected by RT-PCR.Results:Acupotomy release improves localized pathology of the shoulder capsule,reduces synovial and interstitial fibrillar effusion,adhesions and mechanization and promotes tissue repair.Significantly increased IL-6,HLGB1,ASIC3 gene and protein expression levels in the rabbit shoulder synovium in the model group compared with the blank group(all P<0.05).The expression levels of IL-6,HLGB1 and ASIC3 genes and proteins were significantly reduced in the rabbit shoulder synovium in the acupotomy group compared with the model group(all P<0.05).The content of IL-6,HLGB1,and ASIC3 proteins in the model group was significantly increased and the gene expression level was significantly up-regulated,which was significantly different from the blank group(all P<0.05),significantly lower protein content of the above indicators in the acupotomy group,and has a significant inhibitory effect on the expression level of these genes,significantly different compared to the model and blank group(all P<0.05).Conclusion:Acupotomy therapy reduces IL-6,HLGB1,ASIC3 protein expression levels in the shoulder bursa synovium,this may be one of the mechanisms by which acupuncture works effectively in treating scapulohumeral periarthritis.
