摘要:
目的:探讨半夏泻心汤治疗胃黏膜异型增生(GED)的临床疗效,并分析其对多发性肿瘤抑制因子1(p16)和染色体10上缺失的磷酸酶与张力蛋白同源物基因(PTEN)甲基化的影响。方法:将92例GED患者按随机数字表法分为对照组、观察组各46例。两组均进行西医常规对症治疗,观察组加服半夏泻心汤,治疗3个月后,对比两组治疗前后的中医症候评分并据此评估其疗效。抽取观察组治疗前后外周静脉血提取DNA,采用甲基化特异性PCR(MSP)检测治疗前后p16、PTEN基因甲基化情况,比较观察组治疗前后p16、PTEN基因甲基化变化。结果:治疗后,两组中医症候各条目评分均较治疗前降低,且观察组均低于对照组(P<0.05); 观察组临床有效率82.61%高于对照组54.35%(P<0.05); 观察组p16、PTEN基因甲基化率均较治疗前降低(P<0.05)。结论:半夏泻心汤治疗GED患者可明显减轻临床症状,提高治疗效果,且可抑制p16、PTEN基因的甲基化,从而延缓了GED患者病情向癌变进展。
Abstract:
Objective:To investigate the clinical therapeutic efficacy of Banxia Xiexin decoction on gastric epithelial dysplasia(GED),and analyze its effects on the gene methylation of multiple tumor suppressor 1(p16)and phosphatase and tensin homolog deleted on chromosome 10(PTEN).Methods:Using random number table method,92 GED patients were divided into the control group(n=46),the observation group(n=46).Both groups were treated with western medicine routine symptomatic treatment,the observation group were added with Banxia Xiexin decoction.After 3 months of treatment,the TCM syndromes scores were compared of the two groups before and after therapy and the efficacy was evaluated accordingly.Venous blood were draw and DNA was extracted in the observation group before and after the therapy,and mythylation states of genes p16 and PTEN were detected in the observation group by methylation special PCR(MSP),and serum p16 and PTEN genes methylation states in the observation group before and after the treatment were compared.Results:After the therapy,the scores of each item of TCM symptoms were lower than before in the two groups,and the observation group was lower than the control group(P<0.05); the clinical effective rate of the observation group was 82.61% higher than that of the control group 54.35%(P<0.05).The methylation rates of p16 and PTEN genes in the group were lower than before(P<0.05).Conclusion:Banxia Xiexin decoction can significantly reduce clinical symptoms in patients with GED,in addition to inhibit the methylation of p16 and PTEN genes,thus effectively delay the canceration of illness of GED patients.
