摘要:
功能性消化不良(FD)是临床常见的身心疾病之一,近年来发病率逐渐增加,本病的发病机制尚未明确,目前已有较多研究致力于FD的临床及相关基础研究。西医多从胃肠的动力障碍入手,中医多认为本病由于肝郁脾虚,但临床中发现多数患者存在因精神心理因素复发的情况,中医脏腑体系认为心为神明之大主,可以调节神志活动,并且参与了FD的发病过程。在治疗上,西医主要采用对症处理,中医药疗法在本病的防治上具有明显的特色,其中,针灸作为中医特色疗法,在本病的治疗中具有一定的运用优势,得到临床的认可。本文通过理论分析及总结导师临证经验得出,在治疗FD中,健脾和胃与调养心神合用,以八脉交会穴之内关、公孙为主穴并配合腹部腧穴及百会,同时配合中药,并随诊加减,可获良效。在心胃相关理论指导下,针药并用可使得FD的治疗方法更加丰富,发挥中医药的特色。
Abstract:
Functional dyspepsia(FD)is one of the common physical and mental diseases in clinical practice.In recent years,the incidence of FD has been increasing gradually.The pathogenesis of FD has not been clarified yet,and many studies have been devoted to the clinical and related basic studies.Western medicine usually starts with the dynamic barrier of stomach and intestine,and Chinese medicine usually thinks that the disease is caused by liver depression and spleen deficiency,but clinically,it is found that most patients have relapsed due to mental and psychological factors.The zangfu system of Chinese medicine believes that the heart is the great master of mind,which can regulate mental activities and participate in the onset process of FD.In terms of treatment,western medicine mainly adopts symptomatic treatment,and TCM therapy has obvious characteristics in the prevention and treatment of this disease.Among them,acupuncture and moxibustion,as a characteristic therapy of TCM,has certain application advantages in the treatment of this disease and has been recognized clinically.In this paper,through theoretical analysis and summary of the clinical experience of the instructor,it is concluded that in the treatment of FD,it is necessary to strengthen the spleen and stomach and nourish the mind.In addition,in combination with traditional Chinese medicine,good effects can be obtained by focusing on Neigun and Gongsun acupoints of eight arteries and combining them with abdominal acupoints and Baihui.Under the guidance of relevant theories of heart and stomach,the combination of acupuncture and medicine can enrich the treatment methods of FD,so as to give full play to the characteristics of traditional Chinese medicine.
