摘要:
目的:研究探讨柴芩承气汤联合早期肠内营养治疗重症胰腺炎及对患者淀粉酶(AMY)、脂肪酶(LIPA)、乳酸脱氢酶(LDH)及免疫功能的影响。方法:以回顾性分析的方法选取80例重症胰腺炎患者,按患者治疗方案不同分为观察组、对照组,各40例。对照组以早期肠内营养治疗干预,观察组以柴芩承气汤联合早期肠内营养治疗。综合评估患者的临床疗效、AMY、LIPA、LDH水平、炎症因子水平、免疫功能指标水平、治疗安全性。结果:观察组总有效率92.50%(37/40),对照组患者总有效率75.00%(30/40),观察组高于对照组(P<0.05)。治疗后观察组AMY、LIPA、LDH水平均低于对照组(P<0.05)。治疗后观察组炎症因子包括Toll样受体(TLR4)、白介素-7(IL-7)、单核细胞趋化因子-1(MCP-1)水平均低于对照组(P<0.05)。观察组CD3+、CD4+、CD4+/CD8+均高于对照组,CD8+则低于对照组(P<0.05)。治疗期间观察组、对照组不良反应发生率比较,差异无统计学意义。结论:柴芩承气汤联合早期肠内营养治疗重症胰腺炎的疗效显著,能够降低AMY、LIPA、LDH和炎症因子水平,改善患者的免疫功能,治疗安全性好。
Abstract:
Objective:To study the effect of Chaiqin Chengqi decoction combined with early enteral nutrition on amylase(AMY),lipase(LIPA),lactate dehydrogenase(LDH)and immune function in patients with severe pancreatitis.Methods:80 patients with severe pancreatitis were selected and divided into observation group and control group according to different treatment schemes,40 cases in each group.And the observation group was treated with Chaiqin Chengqi decoction combined with early enteral nutrition.The clinical efficacy,AMY,LIPA,LDH levels,inflammatory factors,immune function index levels and treatment safety were comprehensively evaluated.Results:After treatment,the levels of AMY,LIPA and LDH in the observation group were lower than those in the control group(P<0.05).After treatment,the levels of inflammatory factors including TLR4,IL-7 and MCP-1 were lower than those in the control group(P<0.05).During the treatment,the incidence of adverse reactions in the observation group and the control group was 15.00% and 10.00%(P>0.05).Conclusion:Chaiqin Chengqi decoction combined with early enteral nutrition in the treatment of severe pancreatitis has significant effect,can reduce the levels of AMY,LIPA,LDH and inflammatory factors,improve the immune function of patients,has good safety.
