摘要:
目的:探讨补阳还五汤在面部缺损皮瓣修复术后的恢复疗效及安全性。方法:选取面部皮肤软组织缺损患者100例,按随机数字表法分为对照组和观察组各50例,所有患者均行皮瓣修复术,对照组术后常规抗感染、抗凝及抗血管痉挛治疗; 观察组在对照组基础上连续两个月口服补阳还五汤; 术后当日和随访3～6个月时检测两组血液流变学指标和皮瓣肿胀程度,评估恢复疗效和术后并发症。结果:两组患者均顺利完成手术,未发生静脉危象,术后当日两组主要血液流变学指标比较差异均无统计学意义(P>0.05); 随访3～6个月时观察组患者主要血液流变学指标、皮瓣肿胀度、肿胀消除时间和术后并发症均少于对照组(均P<0.05); 两组恢复疗效相当,但观察组患者的痊愈率高于对照组,差异有统计学意义(P<0.05)。结论:补阳还五汤具有活血化瘀、通络消肿、软坚溃脓的独特功效,面部缺损皮瓣修复术后采用中西医结合疗法更有利于提高皮瓣成活率、消除肿胀程度,缩短肿胀时间,改善术后恢复效果。
Abstract:
Objective:To explore the recovery effect and safety of Buyang Huanwu decoction after repairing facial defect skin flaps.Methods:A total of 100 patients with facial skin and soft tissue defects were selected and randomly divided into the control group and the observation group with 50 cases in accordance with the number table.All patients underwent flap repair.The control group received routine anti-infection,anti-vasospasm treatment and anticoagulation after surgery.The observation group added Buyang Huanwu decoction for two consecutive months on the basis of the control group.On the postoperative day and during the follow-up period of 3 to 6 months,the two groups of blood rheology indexes and the degree of skin flap swelling were measured.Evaluate the recovery effect and postoperative complications.Results:Both groups of patients successfully completed the operation without venous crisis.There was no significant difference in the main hemorheological indexes between the two groups on the postoperative day(P>0.05).The main blood of the observation group was followed up for 3 to 6 months,the rheological indexes,flap swelling degree,swelling elimination time and postoperative complications were all less than those of the control group(all P<0.05).The two groups had the same recovery effect,but the recovery rate of the observation group was higher than that of the control group,and there were differences statistically significant(P<0.05).Conclusion:Buyang Huanwu decoction has the unique effect of promoting blood circulation,removing blood stasis,dredging collaterals,reducing swelling,softening and ulcerating pus.The use of integrated traditional Chinese and western medicine after the repair of facial defect flaps is more beneficial to increase the survival rate of the flaps,eliminate swelling,and shorten the swelling time,improve the postoperative recovery effect.
