摘要:
目的:探讨护胃汤配合西药预防Ⅲ期胰腺癌放疗后消化道黏膜放射性损伤疗效及潜在机制。方法:选取接受放疗预防Ⅲ期胰腺癌患者共116例,以随机抽签法分为两组,其中对照组58例采用单纯西药预防,试验组58例则在西药基础上加用护胃汤预防,比较两组临床疗效、放疗后消化道黏膜损伤程度分级、症状体征评分、促炎细胞因子及氧化应激指标水平。结果:试验组治疗总有效率显著高于对照组(P<0.05); 两组放疗后Ⅰ-Ⅱ级消化道黏膜放射性损伤发生率比较差异无统计学意义(P>0.05); 试验组放疗后Ⅲ-Ⅳ级消化道黏膜放射性损伤发生率均显著低于对照组(P<0.05); 两组放疗后14 d恶心呕吐、食欲下降、腹痛及血便评分均显著高于放疗后7 d(P<0.05); 试验组放疗后7 d和14 d恶心呕吐、食欲下降、腹痛及血便评分均显著低于对照组(P<0.05); 两组放疗后14 d 白介素-1β(IL-1β)、白细胞介素-6(IL-6)、白细胞介素-8(IL-8)、肿瘤坏死因子-α(TNF-α)水平均显著高于放疗后7 d(P<0.05); 试验组放疗7 d和14 d后IL-1β、IL-6、IL-8及TNF-α水平均显著低于对照组(P<0.05); 试验组放疗后14 d 超氧化物歧化酶(SOD)水平显著高于对照组(P<0.05); 试验组放疗后14 d 丙二醛(MDA)水平显著低于对照组(P<0.05)。结论:自拟护胃汤联合西药预防Ⅲ期胰腺癌放疗后消化道黏膜放射性损伤可有效缓解黏膜损伤程度,改善临床症状; 而该方案对于患者血清促炎细胞因子表达和氧化应激损伤的抑制作用可能是疗效更佳关键机制所在。
Abstract:
Objective:To investigate the clinical effect and mechanism of Huwei decoction combined with western medicine in preventing radiation injury of digestive tract mucosa in patients with Ⅲ stage pancreatic cancer after radiotherapy.Methods:116 patients with Ⅲ stage pancreatic cancer after radiotherapy were randomly divided into two groups including control group(58 patients)with western medicine used alone and experiment group(58 patients)with Huwei decoction on the basis of control group; and the gastrointestinal mucosal injury grade,the symptoms and signs score and the levels of proinflammatory cytokines after radiotherapy of two groups were compared.Results:The total effective rate of experiment group were significant higher than control group(P<0.05).There was no significant difference in the incidence of grade Ⅰ-Ⅱ gastrointestinal mucosal radiation injury after radiotherapy between two groups(P>0.05).The incidence of grade Ⅰ-Ⅱ gastrointestinal mucosal radiation injury after radiotherapy of experiment group were significant less than control group(P<0.05).The scores of nausea and vomiting,loss of appetite,abdominal pain and bloody stool in 14 d after radiotherapy of both groups were significantly higher than 7 d after radiotherapy(P<0.05).The scores of nausea and vomiting,loss of appetite,abdominal pain and bloody stool in 7 d and 14 d after radiotherapy of experiment group were significantly lower than control group(P<0.05).The levels of IL-1β,IL-6,IL-8 and TNF- α of 2 groups in 14 d after radiotherapy of both groups were significantly higher than 7 d after radiotherapy(P<0.05).The levels of IL-1β,IL-6,IL-8 and TNF-α in 7 d and 14 d after radiotherapy of experiment group were significantly lower than control group(P<0.05).The levels of SOD in 14 d after radiotherapy of experiment group were significantly higher than control group(P<0.05).The levels of MDA in 14 d after radiotherapy of experiment group were significantly lower than control group(P<0.05).Conclusion:Huwei decoction combined with western medicine in preventing radiation injury of digestive tract mucosa in patients with advanced pancreatic cancer after radiotherapy can significantly alleviate the mucosal injury degree and improve clinical symptoms; and the better inhibition of serum pro-inflammatory cytokine expression and oxidative stress injury may be the key mechanism for better efficacy.
