摘要:
目的:探讨小柴胡汤加减对代谢相关性脂肪性肝病患者肝功能及胰岛素抵抗的影响。方法:将70例代谢相关性脂肪性肝病患者,按随机数字表法分为对照组(35例,给予水飞蓟宾胶囊治疗)和研究组(35例,在对照组治疗基础上给予小柴胡汤加减治疗)。观察治疗前后两组中医证候积分、肝功能、胰岛素抵抗指数,统计临床疗效。结果:治疗前,两组胸胁胀闷、郁郁微烦、身体困重、腹胀不适、时欲太息、恶心欲吐、食少纳呆中医证候评分及γ-谷氨酰基转移酶(γ-GT)、脂肪肝指数(FLI)、天冬氨酸转氨酶(AST)、碱性磷酸酶(ALP)、丙氨酸氨基转移酶(ALT)、空腹血糖(FBG)、空腹胰岛素(FINS)、胰岛素抵抗指数(HOMA-IR)水平比较差异均无统计学意义(P>0.05); 治疗后,两组胸胁胀闷、郁郁微烦、身体困重、腹胀不适、时欲太息、恶心欲吐、食少纳呆中医证候评分及γ-GT、FLI、AST、ALP、ALT、FBG、FINS、HOMA-IR水平均较治疗前降低,与对照组相比,研究组胸胁胀闷、郁郁微烦、身体困重、腹胀不适、时欲太息、恶心欲吐、食少纳呆中医证候评分及γ-GT、FLI、AST、ALP、ALT、FBG、FINS、HOMA-IR水平较低,临床疗效更高,差异有统计学意义(均P<0.05)。结论:小柴胡汤加减可有效改善代谢相关性脂肪性肝病患者肝功能,降低胰岛素抵抗水平,疗效显著。
Abstract:
Objective:To explore the effect of modified Xiaochaihu decoction on liver function and insulin resistance in patients with metabolic-related fatty liver disease.Methods:Seventy patients with metabolic-related fatty liver disease were divided into the control group(35 cases,treated with silybin capsule)and the study group(35 cases,on the basis of the treatment of the control group,modified Xiaochaihu decoction was given with treatment).Observed the scores of TCM syndromes,liver function,and insulin resistance index in the two groups before and after treatment,and made statistics of clinical efficacy.Results:Before treatment,there was no significant different between the two groups of chest and hypothermia distended,depression and slight annoyance,body sleepy and heavy,abdominal distension and discomfort,excesstive rest,nausea,poor appetite TCM syndrome score and γ-glutamyl transferase(γ-GT),fatty liver index(FLI),aspartate aminotransferase(AST),alkaline phosphatase(ALP),alanine aminotransferase(ALT),fasting blood glucose(FBG),fasting insulin(FINS),insulin resistance index(HOMA-IR)(P>0.05).After treatment,the two groups of chest and hypotension,depression and slight annoyance,body sleepy and heavy,abdominal distension,discomfort,excessive rest,nausea,poor appetite TCM syndrome score and γ-GT,FLI,AST,ALP,ALT,FBG,FINS,and HOMA-IR levels were lower than before treatment.Compared with the control group,the study group had chest swelling and tightness,depression and slight annoyance,sleepy and heavy,abdominal distension and discomfort,and restlessness,nausea,poor appetite TCM syndrome score and γ-GT,FLI,AST,ALP,ALT,FBG,FINS,HOMA-IR had lower levels and higher clinical efficacy,the difference was statistically significant(all P<0.05).Conclusion:Xiaochaihu decoction can effectively improve the liver function of patients with metabolic-related fatty liver disease and reduce the level of insulin resistance,the curative effect is remarkable.
