摘要:
目的:研究温肾健脾清化汤辅助治疗脾肾气虚型慢性肾脏病(CKD)的临床疗效及对血清转化生长因子β1(TGF-β1)、纤维连接蛋白(FN)水平的影响。方法:选取脾肾气虚型CKD患者96例,按照随机数字表法分为常规组和试验组,各48例。常规组采用对症治疗及常规西药治疗,试验组在常规组基础上加服温肾健脾清化汤治疗,均持续治疗6个月。比较两组治疗6个月后临床疗效,治疗前、治疗6个月后中医症候积分、肾功能指标[血肌酐(SCr)、估算肾小球滤过率(eGFR)、24 h尿蛋白定量(24 h UPro)]、血清肾纤维化指标(TGF-β1、FN),并统计治疗期间不良反应。结果:治疗6个月后两组中医症候积分低于治疗前,且试验组低于常规组(P<0.05); 试验组总有效率89.58%高于常规组72.92%(P<0.05); 治疗6个月后两组SCr、24 h UPro低于治疗前,eGFR高于治疗前,且试验组SCr、24 h UPro低于常规组,eGFR高于治疗前(P<0.05); 治疗6个月后两组TGF-β1、FN低于治疗前,且试验组低于常规组(P<0.05); 两组不良反应发生率比较差异无统计学意义(P>0.05)。结论:温肾健脾清化汤辅助治疗脾肾气虚型CKD临床疗效显著,能降低中医证候积分,改善肾功能及肾纤维化指标,安全性高。
Abstract:
Objective:To study the clinical efficacy of Wenshen Jianpi Qinghua decoction in the treatment of chronic kidney disease(CKD)with deficiency of spleen and kidney qi and its effect on serum transforming growth factor β1(TGF-β1),fibronectin(FN)levels.Methods:Ninety-six patients with CKD of deficiency of spleen and kidney qi were randomly divided into general group(n=48)and experimental group(n=48).The general group was treated with symptomatic treatment and conventional western medicine.The experimental group was treated with Wenshen Jianpi Qinghua decoction on the basis of the general group,which lasted for 6 months.The clinical efficacy of the two groups after 6 months of treatment,TCM syndrome scores,renal function indexes [serum creatinine(SCr),estimated glomerular filtration rate(eGFR),24 h urine protein quantitative(24 h UPro)],serum renal fibrosis indicators(TGF-β1,FN),and statistical adverse reactions during treatment.Results:After 6 months of treatment,the TCM syndrome score of the two groups was lower than that before treatment,and that of the experimental group was lower than that of the general group(P<0.05).The total effective rate of the experimental group was 89.58%,which was higher than 72.92% in the general group(P<0.05).After 6 months of treatment,SCR and 24 h Upro in the two groups were lower than before treatment,eGFR was higher than before treatment,and SCR and 24 h Upro in the experimental group were lower than those in the general group,eGFR was higher than before treatment(P<0.05).After 6 months of treatment,TGF-β1,FN was lower than that before treatment,and the experimental group was lower than that in the general group(P<0.05).There was no difference in adverse reactions between the two groups(P>0.05).Conclusion:Wenshen Jianpi Qinghua decoction has significant clinical effect in the treatment of CKD with spleen and kidney qi deficiency.It can reduce the score of TCM syndrome,improve renal function and renal fibrosis index,and has high safety.
