摘要:
目的:探讨四物消风饮加味联合氯雷他定片治疗特应性皮炎(AD)血虚风燥证的临床疗效以及对辅助性T细胞1(Th1)/辅助性T细胞2(Th2)平衡的影响。方法:选取124例AD患者按随机数字表法平均分配至对照组和观察组,各62例。对照组予氯雷他定片,观察组于对照组的基础上予四物消风饮加味内服。两组治疗时间为8周。比较两组AD皮损评分指数(SCORAD)与视觉模拟量表(VAS)评分、皮肤屏障功能、临床疗效、外周血Th1和Th2细胞水平、血清干扰素(IFN)-γ与白细胞介素(IL)-4水平。结果:治疗后,两组SCORAD、VAS评分明显减少,且观察组明显低于对照组(P<0.01); 治疗后,两组皮脂含量、角质层含水量明显升高,TEWL显著减少,且观察组以上指标改善更明显(P<0.01); 观察组的总有效率显著高于对照组(P<0.05); 治疗后,两组Th1、Th1/Th2、血清IFN-γ水平明显升高,Th2、IL-4明显降低,且观察组改善更明显(P<0.01)。结论:四物消风饮加味联合氯雷他定片治疗AD血虚风燥证的疗效明显,可有效改善患者的症状体征、皮肤屏障功能,其机制可能与调节Th1/Th2平衡有关。
Abstract:
Objective:To investigate the efficacy of Siwu Xiaofeng decoction combined with loratadine tablets on syndrome of blood deficiency and wind dryness in atopic dermatitis(AD)cases and effect on the balance of T helper 1 cell(Th1)/T helper 2 cell(Th2).Methods:One hundred and twenty-four AD patients were allocated to the control group and the observation group according to the random number table method with 62 cases in each group.Control group was given with loratadine tablets,and the observation group was additionally given with Siwu Xiaofeng decoction based on the control group.Both groups were treated for 8 weeks.Scores of AD skin lesion index(SCORAD)and visual analogue scale(VAS),skin barrier function,the clinical efficacy,peripheral blood levels of Th1 and Th2 cell,and serum levels of interferon(IFN)-γ and interleukin(IL)-4 were compared between the two groups.Results:After treatment,scores of SCORAD and VAS for both groups were obviously decreased,and the observation group was obviously lower as compared with the control group(P<0.01).After treatment,sebum content and cuticle water content in both groups were significantly increased,percutaneous water loss(TEWL)was significantly decreased,and the improvement of above indexes in observation group was significantly better than that in control group(P<0.01).Compared with control group,the total effective rate for observation group was significantly higher(P<0.05).After treatment,Th1,Th1/Th2,and serum level of IFN-γ in the two groups were significantly increased,while Th2 and IL-4 level was significantly decreased,and the improvement of above indexes in the observation group was significantly better(P<0.01).Conclusion:The efficacy of Siwu Xiaofeng decoction combined with loratadine tablets on syndrome of blood deficiency and wind dryness in AD cases is obvious,can effectively improve the symptoms and signs,and skin barrier function of patients,which may be related to the condition Th1/Th2 balance.
