摘要:
目的:观察探讨柴芩承气汤保留灌肠治疗腑实热结型急性胰腺炎(AP)疗效及对中医症候积分、血清淀粉酶和并发症的影响。方法:选取腑实热结型AP患者152例为观察对象,将入选患者通过随机数字表法分为柴芩承气汤组和常规治疗组,每组76例,常规治疗组给予常规治疗,柴芩承气汤组在常规治疗组基础上给予柴芩承气汤保留灌肠,比较两组患者治疗后的临床疗效、中医症候积分、临床相关指标、血清淀粉酶及胃肠激素水平和并发症发生情况。结果:治疗后,柴芩承气汤组临床总有效率为94.74%,常规治疗组为82.21%,两组比较差异具有统计学意义(P<0.05); 两组患者各项中医症候评分及总积分均较治疗前显著降低(P<0.05),且柴芩承气汤组明显低于常规治疗组(P<0.05); 柴芩承气汤组腹痛腹胀消失时间、首次排气排便时间、肠鸣音恢复时间及禁食时间均明显短于常规治疗组(P<0.05); 两组血清淀粉酶水平呈降低趋势(P<0.05),GAS、MTL和5-HT均呈升高趋势(P<0.05),且两组治疗第3天、治疗第7天后以上指标比较差异具有统计学意义(P<0.05); 治疗期间,柴芩承气汤组并发症总发生率(6.58%)明显低于常规治疗组(18.42%),两组比较差异具有统计学意义(P<0.05)。结论:柴芩承气汤保留灌肠治疗腑实热结型AP疗效显著,可有效缓解患者中医症候和临床症状,改善其血清淀粉酶及胃肠激素水平,进而促进肠道功能恢复,减少并发症发生,促进患者康复。
Abstract:
Objective:To observe and explore the efficacy of retention enema with Chaiqin Chengqi decoction in treating acute pancreatitis(AP)of heat stagnation and Fu-organ excess type and its influence on scores of TCM syndromes,serum amylase and complications.Methods:152 patients with AP of heat stagnation and Fu-organ excess type were selected as the observation subjects.The selected patients were divided into Chaiqin Chengqi decoction group and conventional treatment group by the random number table method,with 76 cases in each group.The conventional treatment group was given conventional treatment,and the Chaiqin Chengqi decoction group was given retention enema therapy with Chaiqin Chengqi decoction on the basis of the conventional treatment group.The clinical efficacy,scores of TCM syndromes,clinical related indicators,serum amylase and gastrointestinal hormones and occurrence of complications were compared between the two groups after treatment.Results:After treatment,the total clinical effective rate of Chaiqin Chengqi decoction group was 94.74%,and that of conventional treatment group was 82.21%(P<0.05).The scores of various TCM symptoms and total score in the two groups were significantly reduced compared with those before treatment(P<0.05),and the scores in Chaiqin Chengqi decoction group were significantly lower than those in conventional treatment group(P<0.05).The disappearance time of abdominal pain,disappearance time of abdominal distension,first exhaust time,first defecation time,recovery time of bowel sounds and fasting time were significantly shorter in Chaiqin Chengqi decoction group than those in conventional treatment group(P<0.05).Serum amylase level in the two groups showed a decreasing trend(P<0.05),and the levels of GAS,MTL and 5-HT showed increasing trends(P<0.05),and there were significant differences in the levels between the two groups on the 3 d and 7 d of treatment(P<0.05).During treatment,the total incidence rate of complications of 6.58% in Chaiqin Chengqi decoction group was significantly lower than 18.42% in conventional treatment group(P<0.05).Conclusion:Retention enema with Chaiqin Chengqi decoction has a significant efficacy in the treatment of AP of heat stagnation and Fu-organ excess type.It can effectively relieve the TCM symptoms and clinical symptoms,improve the levels of serum amylase and gastrointestinal hormones,promote the recovery of intestinal function,lower the occurrence of complications.
