摘要:
目的:观察理气化痰方联合西药治疗痰火郁结型神经性耳鸣及对患者微循环和中枢神经递质的影响。方法:将84例痰火郁结型神经性耳鸣患者采用简单随机法分为对照组和观察组,每组42例。对照组给予甲钴胺联合盐酸氟桂利嗪治疗,观察组在对照组基础上给予理气化痰方治疗。比较两组耳鸣程度评分、耳鸣残疾量表(THI)评分、中医症候积分、临床疗效、微循环相关评分、中枢神经递质水平。结果:治疗前,两组甲襞微循环积分、耳鸣程度评分、THI评分、中医症候积分比较差异无统计学意义(均P>0.05)。治疗后,两组甲襞微循环积分、中医症候积分、耳鸣程度评分、THI评分降低,且观察组低于对照组(均P<0.05)。治疗前,两组5-羟色胺(5-HT)、γ-氨基丁酸(GABA)水平比较,差异无统计学意义(均P>0.05)。与治疗前比较,两组治疗后GABA水平升高,5-HT水平降低,且观察组5-HT、GABA水平优于对照组(均P<0.05)。观察组治疗4周后总有效率为88.10%,高于对照组的69.05%,两组比较差异有统计学意义(P<0.05)。结论:理气化痰方联合西药治疗痰火郁结型神经性耳鸣可减轻相关症状,提高疗效,其机制可能与改善患者微循环,调节中枢神经递质有关。
Abstract:
Objective:To observe the effect of Liqi Huatan recipe combined with western medicine on the microcirculation and central neurotransmitter of patients with phlegm-fire stagnation type nervous tinnitus.Methods:A total of 84 patients with phlegm-fire stagnation type nervous tinnitus were divided into control group(n=42)and observation group(n=42)by simple random method.The control group was given mecobalamin combined with flunarizine hydrochloride,while the observation group was given Liqi Huatan recipe on the basis of western medicine in the control group.The tinnitus degree score,tinnitus disability inventory(THI)score,TCM syndrome score,curative effect,microcirculation-related score and central neurotransmitter were compared between the two groups.Results:Before treatment,there was no significant difference in nailfold microcirculation score,tinnitus degree score,THI score and TCM syndrome score between the two groups(all P>0.05).After treatment,the nailfold microcirculation score,TCM syndrome score,tinnitus degree score and THI score in the two groups were decreased,and the observation group was lower than the control group(all P<0.05).Before treatment,there was no significant difference in the levels of 5-hydroxytryptamine(5-HT)and γ-aminobutyric acid(GABA)between the two groups(all P>0.05).Compared with before treatment,the level of GABA increased and the level of 5-HT decreased in the two groups after treatment,and the levels of 5-HT and GABA in the observation group were better than those in the control group(all P<0.05).The total effective rate of the observation group after 4 weeks of treatment was 88.10%,which was higher than that of the control group,the difference was statistically significant(P<0.05).Conclusion:The treatment of phlegm-fire stagnation type nervous tinnitus with Liqi Huatan recipe combined with western medicine can reduce the related symptoms and improve the curative effect.
