摘要:
目的:观察参芪地黄汤联合缬沙坦胶囊对慢性肾小球肾炎(CGN)患者细胞免疫和炎症反应的影响。方法:将60例CGN患者随机分为观察组和对照组,每组30例。对照组给予常规西医治疗,观察组采用参芪地黄汤联合缬沙坦胶囊治疗,两组疗程均为8周。记录两组治疗前后中医证候积分、肾功能指标、免疫功能和炎症因子的变化,统计两组临床疗效。结果:观察组治疗总有效率为90.00%,对照组为70.00%,观察组明显高于对照组(P<0.05)。与治疗前比较,两组治疗后中医证候积分降低,且观察组低于对照组(P<0.05)。观察组治疗后的血肌酐、血尿素氮、24 h尿蛋白定量均较治疗前降低,估测肾小球滤过率(eGFR)明显增加,且治疗后观察组eGFR高于对照组,其余肾功能指标均低于对照组。与治疗前比较,治疗后两组CD3+、CD4+、CD4+/CD8+比值增加,CD8+水平降低(P<0.05),且治疗后的观察组上述指标均优于对照组(P<0.05)。与治疗前比较,治疗后两组血清肿瘤坏死因子-α(TNF-α)、超敏C反应蛋白(hs-CRP)、白介素-17(IL-17)水平均降低,治疗后观察组上述指标均低于对照组(P<0.05)。结论:在西医治疗基础上,加用参芪地黄汤可提高CGN治疗的疗效,不仅可改善临床症状与肾功能,还能有效调节细胞免疫功能与炎症反应,值得临床推荐。
Abstract:
Objective:To observe the effect of Shenqi Dihuang decoction combined with valsartan capsules on cellular immunity and inflammatory reaction in patients with chronic glomerulonephritis(CGN).Methods:60 patients with CGN were randomly divided into observation group and control group,30 cases in each group.The control group was treated with conventional western medicine,and the observation group was treated with Shenqi Dihuang decoction combined with valsartan capsules.The course of treatment in both groups was 8 weeks.The changes of TCM syndrome score,renal function index,immune function and inflammatory factors were recorded before and after treatment,and the clinical efficacy of the two groups was statistically analyzed.Results:The total effective rate of the observation group was 90.00%,the control group was 70.00%,the observation group was significantly higher than the control group(P<0.05).Compared with before treatment,the TCM syndrome score of the two groups decreased after treatment were decreased,and the observation group was lower than the control group(P<0.05).After treatment,the serum creatinine,blood urea nitrogen and 24 urine protein of the observation group were lower than those before treatment,and the estimated glomerular filtration rate(eGFR)was significantly increased.After treatment,the eGFR of the observation group was higher than that of the control group,and the other renal function indexes were lower than those of the control group.Compared with before treatment,CD3+,CD4+,CD4+/CD8+ ratio increased and CD8+ level decreased in the two groups after treatment(P<0.05),and the above indexes in the observation group after treatment were better than those in the control group(P<0.05).Compared with before treatment,the serum tumor necrosis factor(TNF-α),hs-CRP and IL-17 levels of the two groups were significantly lower after treatment.After treatment,the levels of TNF-α, hs-CRP and IL-17 in the observation group were lower than those in the control group(P<0.05).Conclusion:on the basis of western medicine treatment,Shenqi Dihuang decoction can improve the curative effect of CGN treatment,not only can improve clinical symptoms and renal function,but also can effectively regulate cellular immune function and inflammatory reaction,which is worthy of clinical recommendation.
