摘要:
肾性血尿是慢性肾小球肾炎的常见临床表现之一,在治疗上较为棘手,常易反复。本病中医上可属“血尿”“溺血”“溲血”的范畴,杨洪涛教授在长期临证中认为肾性血尿以肺脾肾三脏亏虚为发生发展之本,外感和(或)内生之热邪是其重要使动因素,情志抑郁、枢机不利是其加重与反复的诱发因素,而瘀血是导致其迁延反复的继发性病理因素。“治血四法”中止血、化瘀、宁血、补虚四法涵盖补虚泻实,标本兼顾之意,其契合血尿之病机。杨洪涛教授四法灵活运用,临床往有验效。
Abstract:
Renal hematuria is one of the common clinical manifestations of chronic glomerulonephritis.It is difficult and often repeated in treatment.This disease can be classified into the categories of “hematuria”“drowning” and “bleeding” in the traditional Chinese medicine.Professor YANG Hongtao believes that this disease is the syndrome of deficiency and excess,in which deficiency of lung spleen and kidney is the origin of the occurrence and development of the disease,exogenous and/or endogenous heat evil is an important driving factor,emotional depression and poor axis are the inducing factors for its aggravation and repetition,and blood stasis is the secondary pathological factor leading to the prolongation and repetition of the disease.In the “four methods of treating blood”,the four methods of hemostasis,removing blood stasis,calming blood and tonifying deficiency cover tonifying deficiency and reducing excess,giving consideration to both specimens and symptoms,which is in line with the pathogenesis of hematuria.YANG's method is applied flexibly and has proved effective in clinic.
