摘要:
目的:探讨健脾调神温针灸治疗腹泻型肠易激综合征临床效果及对患者情绪改善和氧化应激水平的影响。方法: 选取86例腹泻型肠易激综合征患者,按照随机数字表法分为对观察组和对照组,各43例。对照组采用常规治疗,观察组在对照组基础上联合健脾调神温针灸治疗,观察两组病情严重程度量表(IBS-SSS)评分、生活质量表(IBS-QOL)评分、汉密尔顿抑郁量表(HAMD)评分、汉密尔顿焦虑量表(HAMA)评分及血清氧化应激指标[丙二醛(MDA)、总抗氧化能力(T-AOC)、血清5-羟色胺(5-HT)]的改善情况,排便情况以及两组患者治疗后的临床效果和不良反应发生的情况。结果:治疗后,观察组的治疗总显效率较对照组有明显升高(P<0.05)。治疗后两组IBS-SSS评分和IBS-QOL评分均降低,且观察组该两项评分均低于对照组(P<0.05)。治疗后两组HAMD、HAMA评分与治疗前相比都有明显降低,且观察组的评分结果更低于对照组(P<0.05)。治疗后,两组患者的血清MDA指标比治疗前均有降低,且观察组该指标低于对照组(P<0.05)。治疗后两组血清T-AOC指标均升高,且观察组该指标高于对照组(P<0.05)。治疗后两组患者的血清5-HT指标均降低,且观察组该指标低于对照组(P<0.05)。治疗后,观察组的IL-6、IL-8、CRP、TNF-α的水平低于治疗前,差异均有统计学意义(P<0.05),且观察组的各指标低于对照组(P<0.05)。两组治疗期间均未出现明显不良反应。结论: 健脾调神温针灸能够调节腹泻型肠易激综合征患者机体氧化应激指标,延缓病情进展,有助于缓解患者抑郁焦虑等负性情绪,提高患者生活质量。
Abstract:
Objective:To explore the clinical efficacy of warm acupuncture and moxibustion with Jianpi Tiaoshen on diarrhea type irritable bowel syndrome and its influence on patients' mood improvement and oxidative stress level.Methods:Eighty-six patients with diarrhea-predominant Irritable Bowel Syndrome(IBS)were randomly divided into observation group(n=43)and control group(n=43).The general group was treated with conventional therapy,and the observation group was treated with acupuncture and moxibustion for invigorating the spleen and regulating the mind,the scores of IBS severity scale(IBS-SSS),IBS quality of life scale(IBS-QOL),Hamilton depression scale(HAMD),Hamilton anxiety rating scale(HAMA)and the indexes of oxidative stress [malondialdehyde(MDA),total antioxidant capacity(T-AOC),improvement of serum serotonin(5-HT)],defecation,and the clinical effects and adverse effects after treatment in both groups.Results:After treatment,the total effective rate of the observation group was significantly higher than that of the control group(P<0.05).After treatment,the scores of IBS-SSS and IBS-QOL in both groups decreased,and the scores in the observation group were lower than those in the control group(P<0.05).After treatment,the scores of HAMD and HAMA in both groups were significantly lower than those before treatment,and the scores in the observation group were lower than those in the control group(P<0.05).After treatment,the serum MDA indexes in both groups were lower than those before treatment,and the indexes in the observation group were lower than those in the control group(P<0.05).After treatment,T-AOC in both groups increased,and the level of T-AOC in the observation group was higher than that in the control group(P<0.05).There were no obvious adverse reactions in both groups during the treatment.After treatment,serum 5-HT indicators decreased in both groups,and this indicator in the observation group was lower than that in the control group(P< 0.05).After treatment,the levels of IL-6,IL-8,CRP and TNF-α were lower than before treatment,both significantly(P<0.05).Conclusion:The application of warm acupuncture and moxibustion with Jianpi Tiaoshen can regulate the indexes of oxidative stress in patients with diarrhea type irritable bowel syndrome and improve the progress of the disease,which help alleviate the negative emotions such as depression and anxiety and improve the quality of life of patients.
