摘要:
目的:观察探讨温经汤加减用于肾虚血瘀型卵巢储备功能减退(DOR)的临床疗效,及对患者子宫动脉血流及卵巢功能的影响。方法:将86例肾虚血瘀型DOR患者,随机均分为两组,对照组采用常规西医治疗,观察组在此基础上加用温经汤加减治疗,治疗3个月经周期后,比较两组临床疗效,并比较治疗前后中医症候积分、月经评分及卵巢功能(包括激素水平、子宫动脉血流参数、T细胞亚群)变化。结果:观察组治疗后主症、次症、舌脉象积分及总积分均低于对照组(P<0.05)。观察组治疗后经色经质评分、月经周期、月经量均低于对照组(P<0.05)。观察组治疗后促黄体生成素(LH)、促卵泡激素(FSH)均低于对照组,雌二醇(E2)、抗缪勒管激素(AMH)均高于对照组(P<0.05)。观察组治疗后子宫动脉血流参数OV、AFC、PSV均高于对照组,PI、RI均低于对照组(P<0.05)。治疗后,观察组T细胞亚群CD3+表达明显比对照组高,CD4+、CD8+表达明显比对照组低(P<0.05); 观察组临床疗效95.3%,明显高于对照组的76.7%(P<0.05)。结论:温经汤加减用于肾虚血瘀型DOR的疗效显著,可明显改善中医症候、月经症状及卵巢功能,优于单用西医激素替代治疗。
Abstract:
Objective:To observe and discuss the clinical effect of Wenjing decoction on ovarian reserve dysfunction(DOR)of kidney deficiency and blood stasis type,and its effect on uterine artery blood flow and ovarian function.Methods:A total of 86 patients with DOR of kidney deficiency and blood stasis type were included in the study object.They were divided into two groups by random number table method.43 patients in the control group were treated with conventional western medicine.The observation group was treated with Wenjing decoction on the basis of this.A total of 3 menstrual cycles were treated.The clinical effects of the two groups were observed and compared,and the scores of TCM symptoms before and after treatment were compared changes in menstrual score and ovarian function(including hormone level,uterine artery blood flow parameters,T cell subsets).Results:After treatment,the main symptoms,secondary symptoms,tongue pulse score,and total score of the observation group were lower than those of the control group(P<0.05).After treatment,the color and quality scores,menstrual cycle and menstrual volume of the observation group were lower than those of the control group(P<0.05).After treatment,luteinizing hormone(LH)and follicle-stimulating hormone(FSH)in the observation group were lower than those in the control group,and estradiol(E2)and anti mullerian hormone(AMH)were higher than those in the control group(P<0.05).After treatment,the uterine artery blood flow parameters OV,AFC,and PSV in the observation group were higher than those in the control group,while PI and RI were lower than those in the control group(P<0.05).After treatment,the expression of CD3+in the T cell subgroup of the observation group was significantly higher than that of the control group,while the expression of CD4+and CD8+were significantly lower than those in the control group(P<0.05); the efficacy efficiency of the observation group was 95.3%,significantly higher than the control group's efficacy efficiency of 76.7%(P<0.05).Conclusion:The modified Wenjing decoction has a significant effect on DOR.It can significantly improve the symptoms of traditional Chinese medicine,menstrual symptoms and ovarian function,and is superior to the hormone replacement therapy of western medicine alone.
