摘要:
目的:探讨针刺结合火针对寒凝血瘀型膝骨性关节炎的疗效及对疼痛介质、即刻镇痛效应的影响。方法:选取108例寒凝血瘀型膝骨性关节炎患者,依照随机数字表法分为对照组和观察组,对照组采用双氯芬酸钠缓释片治疗,观察组在对照组治疗的基础上采用针刺结合火针治疗,两组治疗时间均为1个月。对比两组患者治疗前后临床疗效、寒凝血瘀证中医症状评分、血清疼痛介质水平以及即刻镇痛效应(麦吉疼痛问卷简表评分)。结果:两组比较,观察组总有效率高于对照组(P<0.05); 治疗后两组寒凝血瘀证症状评分下降(P<0.05),两组比较观察组较低(P<0.05); 治疗后两组血清P物质(SP)、前列腺素E2(PGE2)、多巴胺(DA)和5-羟色胺(5-HT)水平均降低(P<0.05),且两组比较观察组较低(P<0.05); 治疗前两组患者疼痛自我感觉指数(PRI)、线段自测定量表评分(VAS)、即刻疼痛强度评分(PPI)比较差异均无统计学意义(P>0.05),治疗后两组PRI、VAS、PPI均降低,且观察组更低(P<0.05)。结论:针刺结合火针用于治疗寒凝血瘀型膝骨性关节炎患者,能够改善临床症状,疗效确切,减少疼痛介质,具有即刻镇痛效应。
Abstract:
Objective:To investigate the curative effect of acupuncture combined with fire on cold coagulation and blood stasis type knee osteoarthritis and its influence on pain mediators and immediate analgesic effect.Methods:A total of 108 patients with cold coagulation and blood stasis type knee osteoarthritis were selected and divided into control group and observation group according to the random number table.The control group was treated with diclofenac sodium sustained release tablets,and the observation group was treated with acupuncture combined with fire needle on the basis of the treatment of the control group.The treatment time of both groups was 1 month.Clinical efficacy,traditional Chinese medicine symptom scores,serum pain mediators level and immediate analgesic effect of cold coagulation and blood stasis syndrome were compared between the two groups before and after treatment(McGill pain questionnaire score).Results:The total effective rate of the observation group was higher than that of the control group(P<0.05).After treatment,TCM symptom scores of cold coagulation and blood stasis syndrome decreased in the two groups,of which the observation group was lower(P<0.05).After treatment,the levels of serum Substance P(SP),prostaglandin E2(PGE2),dopamine(DA)and 5-hydroxytryptamine(5-HT)in the two groups were decreased(P<0.05),of which the observation group were lower(P<0.05).Before treatment,there were no significant differences in pain self-perception index(PRI),visual analogue scale(VAS)and immediate pain intensity score(PPI)between the two groups(P>0.05).After treatment,the PRI,VAS and PPI were all decreased in the two groups,and the observation group was lower(P<0.05).Conclusion:Acupuncture combined with fire acupuncture in the treatment of cold coagulation and blood stasis type knee osteoarthritis patients,can improve clinical symptoms,effective,reduce pain medium,has immediate analgesic effect.
