摘要:
目的:探究疏肝理气养血汤加减治疗胃癌化疗癌因性疲乏(CRF)效果及对患者免疫功能、血清皮质醇的影响。方法:将212例中晚期胃癌气血亏虚证患者按照随机数字表法分为观察组103例、对照组105例。对照组实施常规化疗,观察组在对照组基础上给予疏肝理气养血汤加减。比较两组临床疗效、癌因性疲乏量表(CFS)评分、中医证候评分、免疫功能指标、血清皮质醇(Cor)、不良反应发生情况。结果:治疗后两组CFS评分均降低,且观察组低于对照组,差异有统计学意义(P<0.05)。治疗后两组各项中医证候积分均降低,且观察组低于对照组,差异有统计学意义(均P<0.05)。治疗后CD3+、CD4+、CD4+/CD8+、Cor水平升高,且观察组高于对照组,差异有统计学意义(均P<0.05)。治疗后CD8+水平降低,观察组低于对照组,差异有统计学意义(P<0.05)。治疗后观察组总有效为93.20%高于对照组84.76%,差异有统计学意义(P<0.05)。治疗后观察组总不良反应发生率34.95%低于对照组56.19%,差异有统计学意义(P<0.05)。结论:疏肝理气养血汤加减治疗可有效改善胃癌化疗患者癌因性疲乏状态,提高机体免疫力、血清皮质醇水平,疗效较好。
Abstract:
Objective:To investigate effect of modified Shugan Liqi Yangxue decoction in treatment of cancer-related fatigue(CRF)in patients undergoing chemotherapy for gastric cancer,and influence on immune function and serum cortisol.Methods:212 patients with advanced gastric cancer due to Qixue Kuixu were divided into observation group(n=103)and control group(n=105)by random number table method.The control group were treated with conventional chemotherapy,while the observation group were treated with modified Shugan Liqi Yangxue decoction on this basis.Clinical effects,scores of CFS,scores of TCM,immune function indicators,serum Cor and occurrence of adverse reactions were compared between groups.Results:After treatment,scores of CFS and TCM syndrome in both groups decreased,and above scores in the observation group were lower than those in the control group,difference statistically significant(all P<0.05).After treatment,the levels of CD3+,CD4+,CD4+/CD8+ and Cor increased,and above in the observation group higher than those in the control group,difference statistically significant(all P<0.05).After treatment,the level of CD8+ decreased,and CD8+ in the observation group lower than that in the control group,difference statistically significant(all P<0.05).The total effective rate in the observation group 93.20% was significantly higher than that in the control group 84.76%,difference statistically significant(P<0.05).The total incidence of adverse reactions in the observation group 34.95% was significantly lower than that in the control group 56.19%,difference statistically significant(P<0.05).Conclusion:Modified Shugan Liqi Yangxue decoction can effectively improve CRF in patients undergoing chemotherapy for gastric cancer,and improve their immunity and serum cortisol level,with better therapeutic effects.
