摘要:
目的:研究逍遥散合大黄虫丸对经肝动脉化疗栓塞(TACE)术后的原发性肝癌(PLC)患者气滞血瘀型的治疗效果。方法:将80例肝癌气滞血瘀型患者采用随机数字表法分为观察组和对照组各40例。对照组肝癌患者TACE术后予对症治疗,观察组肝癌患者在对症治疗的基础上联合逍遥散合大黄虫丸进行治疗。经治疗1个月后复查肝功能、凝血功能、肿瘤指标及上腹部影像学检查的变化情况。结果:观察组血清甲胎蛋白(AFP)、糖类抗原199(CA199)、糖类抗原125(CA125)等肿瘤指标,谷丙转氨酶(ALT)、谷草转氨酶(AST)、总胆红素(TBIL)、总蛋白(TP)、白蛋白(ALB)等肝功能指标,凝血酶原时间(PT)、凝血酶原活动度(PTA)、国际标准化比值(INR)、纤维蛋白原(FIB)等凝血功能指标低于对照组,差异具有统计学意义(P<0.05),观察组总有效率75.00%,获益率90.00%,对照组总有效率 32.50%,获益率 57.50%,观察组总有效率高于对照组(P<0.05)。结论:肝癌气滞血瘀型患者在经过TACE术后,在对症治疗的基础上加用中药内服方逍遥散合大黄虫丸具有更好的疗效,逍遥散合大黄虫丸可以加强对肝功能的保护,有效地改善凝血功能,降低肝癌的转移程度。
Abstract:
Objective:To study the therapeutic effect of Xiaoyao San combined with Dahuang Zhechong pill on patients with primary liver cancer(PLC)with qi stagnation and blood stasis after transcatheter arterial chemoembolization(TACE).Methods:A total of 80 cases of liver cancer patients with qi-stagnation and blood-stasis were divided into observation group and control group with 40 cases each by random number table method.Patients in control group received symptomatic treatment after TACE,and patients in observation group received symptomatic treatment combined with Xiaoyao San and Dahuang Zhechong pill.Results:Observation group serum alpha-fetoprotein(AFP),carbohydrate antigen 199(CA199),carbohydrate antigen 125(CA125)and other tumor indexes,alanine aminotransferase(ALT),aspartate aminotransferase(AST),total bilirubin(TBIL),total protein(TP),albumin(ALB)and other liver function indexes,prothrombin time(PT),prothrombin activity(PTA),international standardized ratio(INR),fibrinogen(FIB)and other coagulation function indexes were lower than those of the control group,with statistical significance(P<0.05).The total effective rate of the observation group was 75.00%,the benefit rate was 90.00%,and the total effective rate of the control group was 32.50%.The benefit rate was 57.50%,the total effective rate of observation group was higher than that of control group(P<0.05).Conclusion:For patients with liver cancer with qi-stagnation and blood stasis after TACE,on the basis of symptomatic treatment,the traditional Chinese medicine Xiaoyao San combined with Dahuang Zhechong pill has a better effect.Xiaoyao San combined with Dahuang Zhechong pill can strengthen the protection of liver function,effectively improve the coagulation function and reduce the metastasis degree of liver cancer.
