摘要:
目的:探讨“调筋解结”术对慢性非特异性腰痛(CNLBP)大鼠胶原纤维超微结构的影响及对TGF-β1/ERK/CTGF信号通路的调控作用。方法:将24只SD大鼠随机分为对照组、模型组、针刺组、调筋解结组,每组6只。采用双前肢废用联合寒湿刺激法构建CNLBP模型。针刺组每日针刺腰阳关、委中穴,调筋解结组隔日针刺4处结筋病灶点,均连续干预2周。使用机械测痛仪测量机械疼痛阈,扫描电镜观察胶原纤维超微结构,Western blotting检测结筋转化生长因子-β1(TGF-β1)、细胞外信号调节激酶(ERK)、磷酸化的细胞外信号调节激酶(p-ERK)、结缔组织生长因子(CTGF)、Ⅰ型胶原蛋白(Collagen Ⅰ)表达。结果:与对照组相比,模型组机械痛阈显著降低(P<0.05),胶原纤维排列紊乱松散,且TGF-β1、ERK、p-ERK、CTGF、Collagen I表达均上调(P<0.01);调筋解结组机械痛阈较模型组升高(P<0.05),胶原纤维结构紧密有序,且上述蛋白表达降低(P<0.01)。结论:“调筋解结”术通过抑制TGF-β1/ERK/CTGF通路,改善CNLBP大鼠机械痛觉及结筋组织纤维化。
Abstract:
Objective:Investigate impact of the Tiaojin Jiejie technique on ultrastructure of collagen fibers in rats with chronic nonspecific low back pain(CNLBP) and its regulatory effect on TGF-β1/ERK/CTGF signaling pathway.Methods:A total of 24 SD rats were randomly allocated into four groups:control group,model group,acupuncture group and Tiaojin Jiejie group,with 6 rats in each group.The CNLBP model was induced using method of bilateral forelimb immobilization combined with cold-damp stimulation.In the acupuncture group,acupoints Yaoyangguan and Weizhong were needled once a day.In the Tiaojin Jiejieg group,four nodal points of tendon contracture were needled every other day.All interventions lasted for two consecutive weeks.A mechanical algometer was employed to measure mechanical pain threshold.The ultrastructure of collagen fibers were observed under scanning electron microscope.Western blotting was used to detect protein expressions of transforming growth factor-β1 (TGF-β1),extracellular signal-regulated kinase (ERK),phosphorylated extracellular signal-regulated kinase (p-ERK),connective tissue growth factor (CTGF),and type Ⅰ collagen (Collagen Ⅰ) in the nodal points of tendon contracture.Results:Compared with the control group,mechanical pain threshold in the model group was significantly reduced (P<0.05).The collagen fibers were disordered and loosely arranged.Moreover,the expressions of TGF-β1,ERK,p-ERK,CTGF,and Collagen Ⅰ were all up-regulated (P<0.01).In the Tiaojin Jiejie group,mechanical pain threshold was higher than that in the model group (P<0.05).The structure of collagen fibers was tight and well-organized.Additionally,expressions of the aforementioned proteins were decreased (P<0.01).Conclusion:Tiaojin Jiejie technique can ameliorate the mechanical pain perception and fibrosis of the nodal points of tendon contracture in CNLBP rats by inhibiting TGF-β1/ERK/CTGF signaling pathway.
