摘要:
目的：探讨肌骨超声引导下针刀联合液性扩张治疗肩周炎的临床疗效。方法：采用肌骨超声引导下针刀联合液性扩张治疗肩周炎60例（治疗组），并与同期采用电脑中频联合痛点封闭治疗肩周炎60例（对照组）进行回顾性对比分析。观察两组患者治疗前、后的临床疗效、McGill疼痛量表（MPQ）、采用数字评分法（NRS）、肩关节活动度以及治疗前及治疗3周后血清白细胞介素6（IL-6）、肿瘤坏死因子α（TNF-α）水平。采用临床有效率作为临床疗效评价指标。结果：两组患者治疗后MPQ、NRS评分较治疗前均不同程度降低，且治疗组低于对照组，差异有统计学意义（均P＜0.05）。治疗后两组患者肩关节活动度明显改善，且治疗组优于对照组（P＜0.05）。治疗组总有效率为93.33%，高于对照组的总有效率78.33%，差异有统计学意义（P＜0.05）。两组患者治疗3周后，IL-6、TNF-α水平均降低，且治疗组低于对照组，差异有统计学意义（均P＜0.05）。结论：肌骨超声引导下针刀联合液性扩张序贯治疗肩周炎能够减轻肩关节疼痛，改善肩关节功能活动，减少肩关节炎症因子释放，临床疗效较好。
Abstract:
Objective:To investigate clinical effect of musculoskeletal ultrasound-guided acupotomy combined with liquid dilation in treatment of shoulder periarthritis.Methods:60 cases of periarthritis of shoulder were treated with musculoskeletal ultrasound-guided acupotomy combined with liquid dilation (treatment group).60 cases of periarthritis of shoulder were treated with computer medium frequency combined with pain point occlusion (control group).The clinical efficacy,McGill pain scale (MPQ),digital scoring method (NRS),shoulder joint motion,and serum levels of interleukin-6 (IL-6) and tumor necrosis factor α(TNF-α) before and after treatment were observed in the two groups.Clinical effective rate was used as evaluation index of clinical efficacy.Results:After treatment，MPQ and NRS scores of the two groups were lower than those before treatment,and more significantly in the treatment group(all P＜0.05).After treatment,the motion of shoulder joint of the two groups was significantly improved,and the treatment group significantly better than those in the control group (P＜0.05).The total effective rate in the treatment group was 93.33% higher than that in control group 78.33%,difference statistically significant(P＜0.05).After 3 weeks of treatment,the levels of IL-6 and TNF-α in the two groups were decreased,and the levels in the treatment group were significantly decreased compared with the control group(all P＜0.05).Conclusion:Sequential treatment of scapulohumeral periarthritis with musculoskeletal ultrasound-guided acupotomy combined with liquid dilation can significantly reduce shoulder joint pain,improve shoulder joint functional activities,and release of shoulder joint inflammatory factors,with reliable clinical efficacy.
