摘要:
肠易激综合征（IBS）是临床常见的消化系统疾病，以腹痛或腹部不适为主要症状，伴有排便频率或排便习惯的改变，具有排便异常的特征。现代医学研究发现胃肠动力障碍、内脏高敏感、脑-肠轴调节异常、肠道炎症反应、肠道菌群紊乱、情绪因素等均与IBS-D的发病密切相关。江锋教授认为IBS-D病位多在肝、脾、肾，病机以脾虚失运为本，肝郁气滞为标，肾阳亏虚为渐。其主张“辨病论治”，并在此基础上应用IBS-D通治方——解郁运脾汤疏肝理脾、温通肾阳，在临床中取得良好疗效。此外，江教授还特别重视饮食疗法在脾胃病中的应用，将“辨病论治”与“辨证施膳”相结合，药借食力、寓医于食，进一步提高了临床疗效，为中医药治疗IBS-D提供了有益经验。
Abstract:
Irritable bowel syndrome (IBS) is a commonly encountered functional gastrointestinal disorder in clinical practice,characterized primarily by abdominal pain or discomfort,accompanied by altered bowel frequency or defecation habits,and marked by defecation irregularities.Modern medical research has identified multiple contributing factors to the pathogenesis of diarrhea-predominant IBS,including gastrointestinal motility disorders,visceral hypersensitivity,dysregulation of the brain-gut axis,intestinal inflammation,gut microbiota imbalance,and psychological or emotional disturbances.Professor JIANG Feng holds that the pathological sites of IBS-D mainly involve the liver,spleen,and kidneys.The fundamental pathogenesis lies in spleen deficiency and impaired transportation and transformation,with liver qi stagnation as the superficial manifestation and kidney yang deficiency as a progressive condition.Professor JIANG advocates the principle of “treating diseases based on pattern differentiation”,and on this basis,he applies a general therapeutic formula for IBS-D,Jieyu Yunpi decoction,which soothes the liver,regulates the spleen,and warms kidney yang.This approach has demonstrated favorable clinical efficacy.In addition,Professor JIANG places particular emphasis on the application of dietary therapy in managing spleen and stomach disorders.He integrates disease-based treatment with syndrome-differentiated dietary therapy,thereby leveraging the therapeutic power of food to enhance the effects of medicine and embedding medical principles into diet.This integrative strategy further improves clinical outcomes and provides valuable insights into the traditional Chinese medicine-based treatment of IBS-D.
