摘要:
崩漏是中医妇科常见病证，以非经期阴道不规则出血为特征，量多势急为“崩”，量少淋漓为“漏”，二者常相互转化。本病与现代医学中的功能性子宫出血、子宫肌瘤异常出血等疾病相关，若迁延不愈可导致贫血或休克，危及生命。周铭心教授基于冲任不固的核心病机，结合脾气亏虚与血热郁火的旁从病机，创新性提出“益气清固”治法，即益气摄血、清热凉血、固冲止血三法并行，并拟定方药“三合固冲汤”灵活加减。该方法突破传统“塞流、澄源、复旧”分阶段治疗的局限，强调辨证施治与整体调节，结合脉冲式服药节律，增强疗效的同时减少不良反应。典型病例显示，该法能有效控制出血、调节月经周期，且安全性高、复发率低，体现了中医药治疗崩漏的整体优势与临床价值。
Abstract:
Beng-Lou is a common condition in traditional Chinese medicine gynecology,characterized by irregular non-menstrual vaginal bleeding.Profuse and acute bleeding is termed “Beng”,while scanty,dribbling bleeding is called “Lou”,with the two often transforming into each other.This condition is related to modern medical disorders such as dysfunctional uterine bleeding and abnormal bleeding due to uterine fibroids.If prolonged,it can lead to anemia or shock,endangering life.Professor ZHOU Mingxin,focusing on the core pathogenesis of insecurity of the thoroughfare and conception vessels,along with secondary mechanisms of spleen qi deficiency and blood heat with stagnant fire,innovatively proposed the “Boosting Qi and Clearing to Secure” treatment principle.This approach simultaneously applies three methods:boosting qi to control bleeding,clearing heat to cool the blood,and securing the thoroughfare and conception vessels to stop bleeding.He formulated the prescription “Sanhe Guchong decoction” for flexible modification.This method breaks through the limitations of traditional step-by-step treatment (arresting bleeding,addressing the cause,and restoration),emphasizing syndrome differentiation,holistic regulation,and a pulse-style medication rhythm to enhance efficacy while reducing adverse effects.Typical cases demonstrate that this approach effectively controls bleeding,regulates the menstrual cycle,and offers high safety with a low recurrence rate,highlighting the holistic advantages and clinical value of TCM in treating Beng-Lou.
