摘要:
脊髓损伤(SCI)的主要治疗方式是早期手术减压和糖皮质激素脉冲治疗,但治疗不良反应较大,且很难有效抑制神经炎症引起的继发性损伤,促进神经功能恢复。中药因其不良反应较小受到国内外学者的关注,以中医理论为指导,重点研究SCI发生发展涉及的分子靶点,筛选出具有多靶点、作用范围广、不良反应低的中药,发挥中医药治疗SCI的独特优势。SCI的发生发展与多条信号通路的异常激活或抑制密切相关,PI3K/Akt信号通路激活可抑制SCI相关炎症因子表达,与SCI的发生发展联系密切。中药通过调控PI3K/Akt信号通路,抑制SCI细胞凋亡、抑制炎症反应、抑制星形胶质细胞的活化、促进轴突生长,延缓疾病进程,为SCI的防治提供治疗思路。本研究从PI3K/Akt信号通路在SCI发生发展中的作用,从中药单体化合物、中药复方等方面进行讨论,为SCI的临床研究与药物研制提供新思路。
Abstract:
The primary treatment methods for spinal cord injury (SCI) were early surgical decompression and glucocorticoid pulse therapy.However,these treatments have significant adverse effects and struggle to effectively suppress secondary injuries caused by neuroinflammation or promote neurological function recovery.Traditional Chinese medicine (TCM),noted for its relatively fewer adverse effects,has garnered attention from scholars worldwide.Guided by TCM theory,research has focused on molecular targets involved in the onset and progression of SCI,screening for TCM agents with multi-target actions,broad efficacy,and low adverse effects to leverage the unique advantages of TCM in treating SCI.The development of SCI was closely associated with the abnormal activation or inhibition of multiple signaling pathways.Activation of the PI3K/Akt signaling pathway could suppress the expression of SCI-related inflammatory factors and was closely linked to the pathogenesis of SCI.By modulating the PI3K/Akt signaling pathway,TCM could inhibit SCI-induced apoptosis,suppress inflammatory responses,prevent astrocyte activation,and promote axonal growth,thereby slowing disease progression and offering new therapeutic insights for SCI prevention and treatment.This study discussed the role of the PI3K/Akt signaling pathway in the onset and progression of SCI,exploring TCM monomeric compounds and compound formulations to provide novel perspectives for clinical research and drug development in SCI.
