摘要:
目的：探讨益气聪明汤联合旋牵手法治疗气血亏虚型颈性眩晕（CV）的临床疗效。方法：将94例气血亏虚型CV患者按随机数字表法分为对照和观察组，每组47例。两组均给予常规治疗，对照组加用旋牵手法，观察组在对照组基础上联用益气聪明汤，疗程4周。比较两组治疗前后颈性眩晕症状和功能评估量表（ESCV）评分、颈椎功能障碍指数（NDI）评分、椎-基底动脉血流速度、血液流变学指标及血管活性因子水平，并评估临床疗效与安全性。结果：观察组总有效率（93.62%）显著高于对照组（76.60%，P<0.05）。治疗后，两组ESCV评分、椎-基底动脉血流速度及降钙素基因相关肽（CGRP）水平较治疗前升高（均P<0.05），NDI评分、血液流变学各项指标、内皮素-1（ET-1）及血清神经肽Y（NPY）水平较治疗前降低（均P<0.05），且观察组上述各指标改善幅度优于对照组（均P<0.05）。两组治疗期间安全性指标均未见明显异常，未见严重不良事件。结论：益气聪明汤联合旋牵手法可有效调节气血亏虚型CV患者血管活性因子平衡，提高椎-基底动脉血流速度，优化血液流变学指标，改善颈椎功能，缓解患者临床症状，且安全性高。
Abstract:
Objective:To investigate the clinical efficacy of Yiqi Congming decoction combined with rotary traction manipulation in the treatment of cervical vertigo （CV） of Qi-blood deficiency type.Methods:A total of 94 patients with CV of Qi-blood deficiency type were randomly divided into a control group and an observation group,with 47 cases in each group.Both groups received conventional treatment,with the control group additionally receiving rotational traction manipulation,while the observation group received Yiqi Congming decoction combined with rotational traction manipulation for 4 weeks.The cervical vertigo symptoms and functional assessment scale (ESCV) score,neck disability index (NDI) score,vertebrobasilar artery blood flow velocity,hemorheological indices,and vasoactive factor levels were compared before and after treatment,and clinical efficacy and safety were evaluated.Results:The total effective rate in the observation group (93.62%) was significantly higher than that in the control group (76.60%,P<0.05).After treatment,ESCV scores,vertebrobasilar artery blood flow velocity,and calcitonin gene-related peptide (CGRP) levels increased in both groups (all P<0.05),while NDI scores,hemorheological indices,endothelin-1 (ET-1),and serum neuropeptide Y (NPY) levels decreased (all P<0.05).The improvement amplitude of the above indicators in the observation group is better than that in the control group (all P<0.05).No significant abnormalities in safety indices or severe adverse events were observed in either group during treatment.Conclusion:Yiqi Congming decoction combined with rotary traction manipulation can effectively regulate the balance of vasoactive factors in patients with CV of Qi-blood deficiency type,improve vertebral-basilar artery blood flow velocity,optimize hemorheological parameters,enhance cervical function,alleviate clinical symptoms,and demonstrate high safety.
