摘要:
目的：探讨毫火针联合清肺凉血消痤汤治疗寻常痤疮（AV）的临床效果及对患者皮损情况、血清去氢表雄酮（DHEA）、基质金属蛋白酶1（MMP-1）、核苷酸结合寡聚化结构域样受体蛋白3（NLRP3）水平的影响。方法：选取160例AV患者，随机分为两组，汤剂组给予清肺凉血消痤汤治疗，火针组给予毫火针联合清肺凉血消痤汤治疗。比较两组临床效果、患者皮损情况及血清DHEA、MMP-1、NLRP3水平。结果：火针组总有效率为92.50%，高于汤剂组的78.75%（P<0.05）。治疗后，火针组颜面潮红、颜面油腻、口干口苦、大便干结等症状评分及丘疹、脓疱、粉刺、结节囊肿评分低于汤剂组（均P<0.05）。治疗后，火针组痤疮综合分级系统（GACS）评分低于汤剂组（P<0.05），痤疮特异性量表（Acne-QOL）评分高于汤剂组（P<0.05）。治疗后，火针组血清DHEA、MMP-1、NLRP3及白细胞介素-1β（IL-1β）水平低于汤剂组（均P<0.05）。结论：毫火针联合清肺凉血消痤汤治疗AV能够协同增效，改善皮损情况，提高生活质量，降低炎症损伤。
Abstract:
Objective:To discuss the clinical efficacy of the combination of filiform fire needle and Qingfei Liangxue Xiaocuo decoction in the treatment of acne vulgaris (AV),and its impacts on the skin lesions, serum dehydroepiandrosterone (DHEA),matrix metalloproteinase 1 (MMP-1),and nucleotide binding oligomerization domain like receptor protein 3 (NLRP3) levels in patients.Methods:Totally 160 patients with AV were stochastically divided into two groups.The decoction group was treated with Qingfei Liangxue Xiaocuo decoction,while the filiform fire needle group was treated with a combination of filiform fire needle and Qingfei Liangxue Xiaocuo decoction.The clinical effects,as well as the skin lesions,serum DHEA,MMP-1,and NLRP3 levels of the patients in the two groups were compared.Results:The total effective rate of the filiform fire needle group was 92.50%,which was higher than the 78.75% of the decoction group (P<0.05).After intervention,the filiform fire needle group had lower scores for symptoms such as facial flushing,facial oiliness,dry and bitter mouth,and dry stools,as well as scores for papules,pustules,pimples,nodules,and cysts than the decoction group (all P<0.05).After intervention,the filiform fire needle group had lower Global Acne Grading System (GAGS) scores than the decoction group (P<0.05),and higher Acne-specific quality of life questionnaire (Acne-QOL) scores than the decoction group (all P<0.05).After intervention,the filiform fire needle group had lower levels of serum DHEA,MMP-1,NLRP3,and interleukin-1β (IL-1β) than the decoction group (all P<0.05).Conclusion:The combination of filiform fire needle and Qingfei Liangxue Xiaocuo decoction can synergistically enhance the effect on AV,reduce skin lesions,improve quality of life,and reduce inflammatory damage.
