摘要:
摘 要 目的：探讨益气滋肾活血通络方治疗早中期糖尿病肾病（DN）的临床疗效及作用机制。方法：将50例DN患者随机分为对照组和治疗组各25例。对照组予以控制血糖、血压、血脂、饮食及相关常规治疗；治疗组在对照组治疗的基础上，予以益气滋肾活血通络方，疗程共12周。分别在第0、12周观察中医证候积分及肾功能（尿素氮、肌酐、抑光素C）、血脂（胆固醇、甘油三酯、低密度脂蛋白、高密度脂蛋白）、24 h 尿蛋白定量、超敏反应蛋白（HsCRP）、同型半胱氨酸（HCY）、纤维蛋白原（FBG）。结果：①治疗组中医证候改善的总有效率为88.00%，优于对照组的52.00%，差异有统计学意义（P＜0.01）；②经治疗后治疗组患者BUN、Scr、Cysc、24 h尿蛋白定量明显下降（P＜0.05），对照组Scr、24 h尿蛋白定量明显下降（P＜0.05），且治疗组治疗后Scr、Cysc、24 h尿蛋白定量明显低于对照组，差异具有统计学意义（P＜0.05）；③经治疗后治疗组TC、TG、LDLC、HsCRP、HCY、FBG明显下降（P＜0.05），对照组TC、TG、HsCRP、FBG明显下降（P＜0.05），且治疗后治疗组LDLC、HsCRP、HCY明显低于对照组，差异具有统计学意义（P＜0.05）。结论：益气滋肾活血通络方治疗早中期糖尿病肾病可能通过改善DN患者的低密度脂蛋白、炎症反应和血流动力学，从而有效减少尿蛋白，改善肾功能及临床症状，达到延缓DN进展的目的。
Abstract:
Abstract 〖HT5”SS〗Objective：To explore the clinical effect and mechanism of Yiqi Zishen Huoxue Tongluo decoction on early and medium stage of diabetic nephropathy （DN） .Methods:A total of 50 patients with CRF were randomly divided into control and treatment groups with 25 cases per group．The control group patients were under treatment of the control of diet，serum glucose，blood pressure，blood fat as well as other routine treatments，the treatment group patients were received the Yiqi Zishen Huoxue Tongluo decoction on the basis of the therapy of the control group． The patients were followed up for 12 weeks． The Chinese medicine symptom score and key indicators were observed in the zeroth and twelfth week respectively to evaluate its clinical efficacy．Key indicators include BUN、Scr、Cysc、24hPro、TC、TG、LDLC、HsCRP、HCY、FBG. Results：①The total effective rate of Chinese medicine symptom score （88.00%）in treatment group was significantly better than that in control group（52.00%）．②BUN、Scr、Cysc、24hPro were significantly decreased in the treatment group after treatment．Scr and 24hPro were significantly decreased in the control group after treatment． Scr、Cysc、24hPro in the treatment group were significantly reduced compared with the control group（P＜0.05）．③TC、TG、LDLC、HsCRP、HCY、FBG were significantly decreased in the treatment group after treatment．TC、TG、HsCRP、FBG were significantly decreased in the control group after treatment．LDLC、HsCRP、HCY in the treatment group were significantly reduced compared with the control group（P＜0.05）.Conclusion：The Yiqi Zishen Huoxue Tongluo decoction can effectively improve the inflammatory reaction and hemodynamics in the patients with early and medium of DN，which can effectively reduce proteinuria，improved the renal function and Chinese medicine symptom of patients to delay the progress of DN. 
