摘要:
目的:观察电针颈夹脊穴对神经病理性疼痛大鼠脊髓背角谷氨酸受体N-甲基-D-天冬门氨酸(NMDA)、氨基-3-羧基-5-甲基异唑-4-丙酸(AMPA)及单核细胞趋化蛋白-1受体(CCR2)、趋化因子C-X3-C-基元受体1(CX3CR1)表达的影响。方法:将72只大鼠随机分为空白组、假手术组、模型组、电针组、L-α-氨基乙二酸(LAA)组、LAA+电针组,各12只。除空白组外,其余组大鼠均结扎右侧C7脊神经建立神经病理性疼痛模型(假手术组仅暴露神经,不结扎)并结合鞘内置管处置。空白组正常饲养不予干预; 假手术组与模型组同时间点按照电针组进行绑缚处理,每次20 min,连续14 d; 电针组于术后第7天开始干预,电针双侧C7夹脊穴,每次20 min,1次/d,连续治疗14 d; LAA组于术后第7天开始干预,鞘内注射LAA,5 nmol/次,1次/d,持续14 d; LAA+电针组于术后第7天开始干预,同时间点按照电针组与LAA组进行电针夹脊穴与5 nmol/次鞘内注射LAA。采用免疫荧光法检测各组大鼠C7脊髓节段脊髓背角CXC3R1受体、CCR2 受体的表达量; 采用Real-Time PCR技术检测各组大鼠脊髓背角NMDA、AMPA 受体的表达。结果:与空白组相比,假手术组大鼠脊髓背角NMDA、AMPA、CCR2、CX3CR1表达差异无统计学意义(P>0.05); 与假手术组相比,模型组大鼠脊髓背角NMDA、AMPA、CCR2、CX3CR1表达量均升高(P<0.05); 同模型组相比,电针组、LAA组与LAA+电针组大鼠NMDA、AMPA、CCR2、CX3CR1表达量均降低,差异有统计学意义(P<0.05); 且LAA+电针组相较于电针组、LAA组的NMDA、AMPA、CCR2、CX3CR1的表达量明显降低(P<0.05)。结论:电针颈夹脊穴可抑制神经病理性疼痛大鼠CCR2、CX3CR1活化,影响CC亚族趋化因子配体2(CCL2)、CX3CL1及其受体CCR2、CX3CR1的结合,并降低谷氨酸受体NMDA、AMPA活化,抑制了胶质细胞的活化及中枢敏化,进而减少促炎性细胞因子和生长因子释放,减少神经病理性疼痛发生。
Abstract:
Objective:To observe the effects of electroacupuncture at cervical Jiaji point on glutamate receptor N-methyl-D-Aspartate(NMDA),alpha-amino-3hydroxy-5-methyl-4-isoxazolepropionic acid(AMPA),C-C Chemokine receptor type 2(CCR2),C-X3-C-Primitive receptor 1(CX3CR1)expression.Methods:Seventy-two rats were stochastic divided into blank control group,sham-operated group,model group,electroacupuncture group,L-α-aminoadipate(LAA)group and LAA combined with electroacupuncture group,with 12 rats in each group.The rats in the other groups were ligation of the right C7 spinal nerve to establish a neuropathic pain model(in the sham group,only the nerve was exposed,but no ligation was performed)and treated with intrathecal tube except the normal group.The blank group was fed normally without intervention.The sham operation group and the model group received binding treatment according to the electroacupuncture group at the same time point,20 min each time,for 14 consecutive days.In the electroacupuncture group,intervention was started on the 7th day after the operation.Electroacupuncture was performed at bilateral C7 Jiaji points,20 min each time,once a day,for 14 consecutive days.In the LAA group,intervention was started on the 7th day after surgery,with intrathecal injection of LAA,5 nmol each time,once a day,for 14 days.The LAA combined with electroacupuncture group began intervention on the 7th day after surgery,and received electroacupuncture Jiaji point and intrathecal injection of LAA at the same time point according to the electroacupuncture group and the LAA group.The expression levels of CXC3R1 receptor and CCR2 receptor in spinal dorsal horn of C7 spinal cord segments of rats in each group were detected by immunofluorescence method.Real-time PCR was used to detect the expression of NMDA and AMPA receptors in spinal dorsal horn of rats in each group.Results:Compared with blank group,the expression of NMDA,AMPA,CCR2 and CX3CR1 in spinal dorsal horn of rats(P<0.05).The expressions of NMDA,AMPA,CCR2 and CX3CR1 in the spinal dorsal horn of the model group were higher than those in the sham operation group(P<0.05); the expressions of NMDA,AMPA,CCR2 and CX3CR1 in electroacupuncture group,LAA group and LAA combined with electroacupuncture group were significantly lower than those in model group(P<0.05); the expression levels of NMDA,AMPA,CCR2 and CX3CR1 in LAA combined with electroacupuncture group were significantly lower than those in electroacupuncture group and LAA group(P<0.05).Conclusion:Electroacupuncture at cervical Jiaji point can inhibit the activation of CCR2 and CX3CR1 in rats with neuropathic pain,affect the binding of CCL2 and CX3CL1 and their receptors CCR2 and CX3CR1,reduce the activation of glutamate receptors NMDA and AMPA,inhibit the activation and central sensitization of glial cells,and then reduce the release of pro-inflammatory cytokines and growth factors,reduce neuropathic pain.
