摘要:
目的：观察消囊调经化痰方联合地中海饮食对痰湿型多囊卵巢综合征伴胰岛素抵抗（PCOS-IR）患者的临床疗效。方法：前瞻性选择符合本研究纳入标准的PCOS患者60例为研究对象。通过随机数字表法并结合组间基线资料均衡可比原则分为饮食指导组、中药组、结合组，每组20例。比较三组患者治疗前后的人体成分、糖脂代谢水平、性激素水平、排卵率、中医证候积分。结果：治疗3个月经周期后，三组体重、BMI、腰围、臀围、体脂率、内脏脂肪面积较治疗前下降(均P<0.05)，中药组、结合组基础代谢率较治疗前提高(均P<0.05)。三组治疗后空腹血糖（FPG）、空腹胰岛素(FINS)、胰岛素抵抗指数(HOMA-IR)、总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白(LDL)水平显著降低，高密度脂蛋白(HDL)显著提升(均P<0.05)，结合组FPG、FINS、HOMA-IR、TG、HDL较其他两组改善更为显著（均P＜0.05）。治疗后三组黄体生成素（LH）、睾酮（T）水平明显下降，卵泡刺激素（FSH）明显提升，结合组LH、T水平低于其他两组（均P＜0.05）。治疗后排卵率结合组＞中药组＞饮食指导组，差异有统计学意义(P<0.05)。三组治疗方案均能改善中医证候，其中改善程度结合组＞中药组＞饮食指导组（P＜0.05）。结论：消囊调经化痰方联合地中海饮食能有效减轻PCOS-IR患者体重、改善性激素分泌及调节糖脂代谢水平。
Abstract:
Objective:To observe the therapeutic effects of Xiaonang Tiaojing Huatan formula in combination with mediterranean diet on clinical outcomes in PCOS-IR patients with phlegm-dampness syndrome.Methods:Sixty patients with PCOS who met the inclusion criteria of this study were prospectively selected as the research subjects.The participants were randomly assigned to three groups using a random number method,ensuring balance in baseline characteristics:dietary guidance group,traditional Chinese medicine group,combined group,with 20 participants in each group.The study compared the changes in body composition,glucose and lipid metabolism,hormone levels,ovulation rates,and traditional Chinese medicine symptom scores before and after treatment in the three groups.Results:After three menstrual cycles of treatment,the body weight,BMI,waist circumference,hip circumference,body fat percentage and visceral fat area of the three groups decreased compared with those before treatment (all P<0.05),and the basal metabolic rate of the traditional Chinese medicine group and the combined group increased compared with that before treatment (all P<0.05).After treatment in the three groups,the levels of fasting plasma glucose (FPG),fasting insulin (FINS),insulin resistance index （HOMA-IR）,total cholesterol (TC),triglycerides (TG),and low-density lipoprotein (LDL) decreased significantly,while the levels of high-density lipoprotein (HDL) increased significantly (all P<0.05).The improvement of FPG,FINS,HOMA-IR,TG and HDL in the combined group was more significant than that in the other two groups (all P<0.05).After treatment,the levels of luteinizing hormone (LH) and testosterone (T) in the three groups decreased significantly,while the levels of follicle-stimulating hormone (FSH) increased significantly.The levels of LH and T in the combined group were lower than those in the other two groups (all P<0.05).After treatment,the ovulation rate in the combination group > the traditional Chinese medicine group > the dietary guidance group showed a statistically significant difference (P<0.05).All three groups of treatment regimens could improve the TCM syndromes.Among them,the degree of improvement was in the order of the combined group > the traditional Chinese medicine group > the dietary guidance group (P<0.05).Conclusion:The combination of Xiaonang Tiaojing Huatan formula and the mediterranean diet effectively alleviates weight,improves sex hormone secretion,and regulates glucose and lipid metabolism in patients with PCOS-IR.
