摘要:
目的：探讨消肿定痛合剂对行椎间孔镜下髓核摘除术（PELD）的腰椎间盘突出（LDH）患者术后早期步态、疼痛程度及腰椎功能恢复的临床疗效。方法：选取40例行PELD治疗的LDH患者，将其随机分为观察组（19例）与对照组（21例）。两组均接受同一常规治疗方案（消炎止痛、脱水消肿、规范化康复训练等），观察组在此基础上加用消肿定痛合剂。分别于术前、术后5 d及术后14 d，采用关节运动功能参数检测仪多维动态检测两组步态参数（步长、步频、步速及膝、踝关节活动度），采用视觉模拟评分法（VAS评分）评估腰痛程度，采用日本骨科协会（JOA评分）评估腰椎活动功能，比较两组上述指标差异。结果：术后5 d，两组患者步态参数、VAS评分及JOA评分均较术前有所改善，观察组患侧步长、步频、步速及膝、踝关节活动度、JOA评分改善幅度均优于对照组（均P＜0.05），但两组VAS评分比较，差异无统计学意义（P＞0.05）。术后14 d，两组患者步态参数、VAS评分及JOA评分均较前持续改善；观察组患侧步长、步频、步速、膝踝关节活动度、JOA评分及VAS评分改善均优于对照组，差异有统计学意义（均P＜0.05）。结论：消肿定痛合剂可有效促进PELD术后LDH患者的步态功能恢复，减轻疼痛，改善腰椎活动功能，有助于早期术后康复。
Abstract:
Objective:To investigate clinical efficacy of Xiaozhong Dingtong Heji on early postoperative gait function,pain intensity and lumbar functional recovery in patients with lumbar disc herniation (LDH) undergoing percutaneous endoscopic lumbar discectomy (PELD).Methods:Forty LDH patients undergoing PELD were randomly divided into an observation group (19 cases) and a control group (21 cases).Both groups received the same conventional treatment (including anti-inflammatory and analgesic therapy,dehydration and detumescence,as well as standardized rehabilitation training).On this basis,the observation group were additionally administered Xiaozhong Dingtong Heji.Gait parameters (step length,step frequency,walking speed,and range of motion of the knee and ankle joints) were measured multidimensionally by a joint motion function detector preoperatively,at 5 days and 14 days postoperatively.Low back pain was evaluated using the visual analogue scale（VAS）and lumbar function was assessed using the Japanese orthopaedic association score (JOA).The differences in changes of all indicators between the two groups were compared.Results:At 5 days postoperatively,gait parameters,VAS scores and JOA scores were improved in both groups compared with preoperative baseline.The improvement in step length,step frequency,walking speed,range of motion of the knee and ankle joints on the affected side,and JOA score in the observation group was superior to that in the control group (all P<0.05),whereas no significant difference in VAS score was found between the two groups (P>0.05).At 14 days postoperatively,all indicators continued to improve in both groups.The observation group showed significantly better improvement in step length,step frequency,walking speed,range of motion of the knee and ankle joints,JOA score and VAS score than the control group,with statistically significant differences (all P<0.05).Conclusion:Xiaozhong Dingtong Heji can effectively promote the recovery of gait function,alleviate pain,and improve lumbar mobility in patients after PELD,facilitating early rehabilitation.
